Work includes, but is not limited to, the following approximate quantities: 88 Ton Aggregate Base Course, Type B 1,583 Square Yards Preparation of Base 45 Ton Aggregate Base Repair 13,275 Pounds Bituminous Materials (Prime and Tack Coats) 164 Ton Hot-Mix Asphalt Binder Course, IL-9.5, N50 1,905 Ton Hot-Mix Asphalt Surface Course, IL-9.5, Mix "C", N50 12,809 Square Yards Hot-Mix Asphalt Surface Removal, 1-1/2" 4,233 Square Yards Hot-Mix Asphalt Surface Removal, 2" 704 Square Yards Class D Patches, Special 108 Foot Gutter Removal 106 Foot Concrete Gutter (Special) 511 Square Foot Sidewalk Removal 563 Square Foot Portland Cement Concrete Sidewalk 20 Square Yards Seeding Various Frames, Lids and Valves to be Adjusted Trafic Control and Protection, (Special) Attention is called to the fact that only bid proposals that cover all the WORK will be accepted and read aloud by the Village. Partial bids will not be accepted. Bid proposals shall be accompanied by data suficient for the Village to evaluate the bidder's qualiications to do the work per the standards set forth in Section 35.10 of the Village Code of Ordinances, including but not limited to a list of projects similar in size and scope in which the Bidder was the prime contractor and completed in the past 5 years. References for such projects, including name, position, phone number and if available, e-mail address, shall also be provided with the proposal. In accordance with Section 35.60 of the Village Code of Ordinance, each bid shall include a complete, accurate and truthful listing and description of all decisions, determinations, judgments or other indings within the past two years of any violation of state or federal laws which protect the health, safety or welfare of workers, including, but not limited to, OSHA, FMLA, FLSA, ADA, ADEA, NLRA, the Federal Civil Rights Act, the State Human Rights Act, the State Wage and Hour Law and the Prevailing Wage Act, against it or any entity with whom it is submitting the bid, including joint ventures and partners, and also including parent and subsidiary corporations or entities, and shall further include documentation that such bidder or its employees are participants in at least one apprenticeship and training program approved by and registered with the U.S. Department of Labor's Board of Apprenticeship and Training. The village may waive the apprenticeship requirement if the bidder is in an industry where no such program is available. If the bidder does not have any decisions, determinations, judgments or other indings against it as aforesaid, it shall include a statement to that effect with its bid. Attention is called to the fact that this project is subject to the Illinois Prevailing Wage Law. A copy of the Village Prevailing Wage Ordinance will be included with the bid documents. The Contractor must ensure that employees and applicants for employment are not discriminated against because of their race, color, religion, sex, or national origin. Furthermore, this contract is available to minority and women contractors. Bidders shall actively solicit minority and women subcontractors and shall maintain records of their solicitation process. Attention is called to the fact that this project is subject to the Public Construction Bond Act, 30 ILCS 550/1, and the successful bidder shall be required to provide a performance and payment bond in the amount of the contract in such form and manner as required by that Act. The Village of Riverton reserves the right to reject or accept any or all bids, any or all bid items, or to waive any informalities in the bidding, and/or award process. Bids may be held by the Village of Riverton for a period of 30 days from the date of the opening of Bids for the purpose of reviewing the Bids and investigating the qualiications of Bidders, prior to awarding the Contract.
