Failure to comply with these specifications in their entirety may be grounds for disqualification. Bids must be made on standard proposal forms as provided in the specifications. All bids shall be enclosed in sealed envelopes bearing on the outside thereof the name and address of the bidder and the contract said bid pertains to, addressed and delivered to the Paterson Public Schools, Department of Purchasing, 90 Delaware, 4th floor, Paterson, New Jersey 07503. A copy of the bidder's Business Registration Certificate issued by the NJ Department of Treasury may be provided with the bid proposal at the time of bid opening. If awarded, vendors shall submit a BRC within 10 days of notification. This requirement is mandatory and failure to comply will disqualify the bid proposal. -Bidders shall comply with the required completion and submittal of a Non-Collusion Affidavit. -Bidders are required to comply with the requirements of N.J.S.A. 10:5-31 et seq. and N.J.A.C. 17:27 et seq. -One of the following documents will be accepted: Letter of Federal Approval, Certificate of Employee Information Report or complete AA302/AA201 form.
