Demolition, site work, renovation and addition to a mixed-use development in Axtell, Nebraska. Completed plans call for the addition of a educational facility; for the demolition of a educational facility; for the renovation of a educational facility; for site work for a educational facility; laboratory facility; laboratory facility; laboratory facility; and laboratory facility.
Duval and Associates has been selected as the PCD and will be providing pre-design and consulting services to the School Dis-trict defining certain criteria to be provided to potential Design-Builders in their de-velopment of proposals for a Design-Build contract, and in providing consulting ser-vices to the School District in the interests of the project. The requirements of Letter of Interest are pursuant to Nebraska LB 889 (2008) Po-litical Subdivision Construc-tion Alternatives Act and the terms defined in Nebraska Revised Statute 13-2906, and Board Policy 3043 De-sign-Build Contracts. The performance criteria developed will allow the dis-trict to construct a project of scope to potentially include: Four to six classrooms in-cluding one classroom being a new science lab. The goal of the district is to complete these additions and improvements in one phased addition of approx-imately $2 million, based on the prioritized needs of the district. The district would like to use the completed project as soon as possible but not later than August The District and PCD shall use the following standards when selecting which pro-spective Design-Builders to prequalify and when rat-ing perspective-interested parties: o Ability or professional personnel o Past performance o Willingness to meet time and budget requirements o Recent, current and projected workloads f the Design-Builder o Other information which the District and PCD deem relevant The Design-Build Con-tracts Policy 7060 adopted by Axtell School District in accordance with NEV. REV. STAT. 13-2909 is attached and incorporated by refer-ence. Duval and Associates, as PCD will be ineligible to be included as a provider of any services in a Design-Builder proposal for the project, and the PCD shall not be employed by, or have a financial or other interest in a Design-Builder who will submit a proposal. Prospective Design-Build-ers who have been preap-proved and offered the RFP shall respond to all items in this questionnaire by June 17, 2022. Failure to do so will deem the Design-Build-ers nonresponsive at the discretion of the School Dis-trict. If the prospective De-sign-Builder team is a prime contractor-subcontractor relationship or joint venture, both entities must complete their relevant information on the questionnaire. Design-Builder's Sub-consultant(s) Firm Infor-mation (If the Design-Builder is a contractor, then list the architectural-engineering team. If the Design-Builder is an Architect or Engineer, then list the General Con-tractor): a. Firm names, address-es, phone, fax and email addresses b. Name of primary point of contact C. Proof of liability insur-ance 3. Design-Builder Team and Experience a. Resume of key team members (Project manager, project superintendent, de-sign architect or engineer) 1. Name and title 2. Role in the project 3. Education 4. Professional registra-tion 5. Years of experience b. List of 3 relevant proj-ects on which the De-sign-Builder provided design and/or construction services similar to those requested in this REP. Provide project scope, budget, year com-pleted and form of project delivery (Design-Build, De-sign-Bid-Build, or Construc-tion Management at Risk). Provide graphics or photos if available. Also, list client references for each of the 3 projects (organization, con-tact name, address, phone, fax and email). c. If the Design-Builder subcontracts for design or construction services, list 3 relevant projects of the designer or contractor and information requested in paragraph above. 4. Design Builder's Ap-proach a. Briefly describe the Design-Builder's approach in working with the School District to design and build the proposed project.
