The Alachua County Housing Authority is requesting quotes for the pump out and inspection of 44 tanks and associated drain fields located in the Thistle Hills East and Thistle Hills West neighborhoods of Archer Florida. After pumping out all tanks, the awarded contractor will be responsible for providing a full final report of the current status and life span of the systems along with a recommended plan for addressing the long term viability of the systems and potential options for upgrades over the next 5 to 10 years. Considerations of costs associated with permitting and soil analysis need to be included. To aid in your estimating I have attached the following: 1. Layouts of each neighborhood showing the approximate locations of the tanks, lift station and known drain fields 2. One page listing of the addresses for each unit and OSTDS Work History from the Alachua County Health Department as of 2019 a. Full back-up of tank of tank data up to 2022 is available via our website under the procurement tab https://www.acha-fl.com/procurement/ 3. Maintenance Wage Rate Determination 4. Table 5.1 of HUD Procurement Handbook 7460.8 REV2 5. Form HUD-5370-C General Conditions for Non-Construction Contracts Section II Include with your quote: Copy of Business License and certifications W-9 DUNS Number if you have one Copy of Insurance Proof of your capability to complete job in a timely manner Please note if you are a MBE, WBE, SME, etc.
