Award Details Contract Award Date: Sep 02, 2022 Contract Award Number: 1240LT22C0069 Task/Delivery Order Number: Contractor Awarded Unique Entity ID: TCK7C1U8EMH9 Contractor Awarded Name: MEDVED GLOBAL INC Contractor Awarded Address: Cortez, CO 81321-8805 USA Base and All Options Value (Total Contract Value): $641591.28 SCOPE OF CONTRACT The intent of this project is to provide aggregate and reconstruct 4.4 miles of the South Mineral Road (NFSR 585). Work includes reshaping the roadbed by reestablishing ditches and pulling the road with back in to a standard 22' width by cutting the shoulders and adding acceptable material to the existing surface. The new surface will be constructed with a crown of no less than 4%. Additional work will include cleaning lead-out ditches, cutting the washboards, filling potholes and covering large rocks. Once the approved roadbed is prepared, commercially purchased aggregate consisting of CDOT class 6 1"-minus roadbase material with a PI between 2-9 shall be hauled, placed and compacted using layer placement method 204.11(a)(1) to a 4" depth and a 22' width. The South Mineral Road will remain open to public traffic during this contract. Flaggers will be required at all times to manage and control traffic. PROJECT LOCATION From the Columbine Ranger District Office, take S. Pearl Street to US160. Turn left onto US 160 west towards Durango for 18.7 miles where US160 and US 550 COMBINE. Continue on US55O towards Silverton for 48.2 miles where US550 turns left to stay on US550 for 2.0 miles. At the intersection of Forest Service Road 585 the project will begin and continue for 4.4 miles to the South Mineral Campground Intersection. MAGNITUDE OF CONSTRUCTION PROJECT Between $500,000 and $1,000,000 C.4 PERIOD OF PERFORMANCE The project contract duration is estimated to have period of performance [POP] of 30 calendar days from the issuance of the Notice to Proceed [NTP]. C.5 Equipment Cleaning & Weed-Free Certification To prevent the introduction of seeds of noxious weeds onto National Forest land, the Contractor shall ensure all equipment moved onto National Forest land is free of soil, seeds, vegetative matter, or other debris that could contain or hold seeds. Contractor shall employ whatever cleaning methods necessary to ensure compliance with this provision and shall notify the Forest Service prior to moving each piece of equipment onto National Forest land. Upon request of Forest Service, arrangements will be made for Forest Service to inspect each piece of equipment prior to being moved onto National Forest land. If the project area is known to contain noxious weeds, the equipment shall be cleaned before moving to other Forest Service system lands which do not contain noxious weeds. If operating in areas known to contain noxious weeds, the contractor shall ensure that prior to moving from one on-forest location to another part of the project area, all off-road equipment is free of soil, seeds, vegetative matter, or other debris that could contain or hold seeds. Contractor shall certify in writing to the Contracting Officer that off-road equipment is free of noxious weeds prior to each start-up of operations and for subsequent moves of equipment to other project areas. Measures taken to ensure that off-road equipment is free of noxious weeds will be identified. "Off-road equipment" includes all equipment operating off paved roadways. A Current list of noxious weeks of concern to Forest Service is available at the Forest Supervisor's office. Contractor shall employ whatever cleaning methods are necessary to ensure that off-road equipment is free of noxious weeds. Equipment shall be considered free of soil, seeds, and other such debris when a visual inspection does not disclose such material. Disassembly of equipment components or specialized inspection tools is not required. Contractor shall notify Forest Service at least 24 hours prior to beginning of work in area designated as a noxious weed area, unless otherwise approved by CO. Notification shall include the location of the equipment's most recent operations. If the prior location of the off-road equipment cannot be identified, Forest Service may assume that it was infested with noxious weed seeds. Upon request by the Contracting Officer, the Contractor must make arrangement for inspection of each piece of off-road equipment prior to it being placed into service. If Contractor desires to clean off-road equipment on National Forest land, such as at the end of a project or prior to moving to a new unit that is free from noxious weeds, Contractor and Contracting Officer shall agree on methods of cleaning, locations for the cleaning, and control of off-site impacts, if any. New infestations of noxious weeds within the project area or on the haul route, shall be promptly reported by the Contractor or Forest Service. All off-road equipment used on this project shall be washed before moving into the project area so that the equipment is free of soil, seeds, vegetative material, or other debris that could contain or hold seeds of noxious weeds. "Off-road equipment" includes all logging and construction equipment and such brushing equipment as brush hogs, masticators, and chippers; it does not include log trucks, chip vans, service vehicles, water trucks, pickup trucks, and similar vehicles not intended for off-road use. Contractor shall clean all vehicles and equipment before entry into the District unless Forest Service waives the cleaning requirement in writing. Thereafter, all vehicles and equipment that leave the district must be cleaned prior to their reentry unless waived by the Forest Service. All cleaning of vehicles or equipment shall be done at sites agreed to by Forest Service and the Contractor. Forest Service shall inspect all equipment prior to entry, as appropriate and determined necessary. Cleaning shall include the removal of soil by steam cleaning or use of high-pressure water spray. Equipment will be considered clean when visual inspection does not reveal soil, seeds, plant material, or other such debris. Disassembly of equipment components or specialized inspection equipment is not required. Contractor shall notify the Contracting Officer at least five days in advance of moving equipment in so that arrangements can be made for inspection. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
