Academy Street Williamstown, New Jersey 08094 lschulz2@monroetwp.k12.nj.us NOTICE REQUEST FOR QUALIFICATIONS FOR ARCHITECT April 29, 2022 pursuant to the provisions of N.J.S.A. 19:44A-20, New Jersey Pay to Play, and other legislative enactments, more specifically Chapter 271 of the laws of the State of New Jersey, the Monroe Township Board of Education located at 75 East Academy Street, Williamstown, New Jersey, 08094, is seeking RFQs for professional services to be provided to the Monroe Township Board of Education as listed below for the period of July 1, 2022 through June 30, 2023. Please review the directions below before submitting proposals. All questions concerning this notice should be addressed in writing to Lisa Schulz, Business Administrator/Board Secretary, at lschulz2@monroetwp.k12.nj.us no later than May 10, 2022 at 11:00 a.m. All responses shall be reviewed immediately after the deadline. Appointment will be awarded at the June 2022 Regular Meeting of the Board of Education (TBD). All proposals must contain an Affirmative Action Certificate of Employee Information Report pursuant to N.J.A.C. 17:27-1.1, a Political Contribution Disclosure Form pursuant to N.J.S.A. 19:44A-20.23, a State of New Jersey Business Registration Certificate pursuant to P.L. 2004, c.57, Disclosure of Investment Activities in Iran pursuant to public law 2012, c.25, and Mandatory Equal Employment Opportunity Language pursuant to N.J.S.A. 10:5-31 et seq. Forms are available on the website. 1. At a minimum, responses must include the following: a. Full name and business address. b. Proof of licensure where applicable. c. Number of licensed professionals employed (if a professional firm). A description of each individuals qualifications, including education, licensure, and years of professional experience. aa. Identify the person who will be primarily responsible for the services required by the Monroe Township Board of Education and provide a description of the experience of the primary person with projects and issues similar to those more specifical ly set forth in this proposal and on behalf of the Monroe Town ship Board of Education. bb. Identify persons who will serve as back up to the primary per son including resumes of all parties. d. A reference listing of current and/or previously served Boards of Education to include contact name, title and phone number. e. Hourly rate of individuals who will perform the services. This rate should be an inclusive rate. The Board will not reimburse for any meals or travel. f. Proof of appropriate professional liability coverage g. Respondent shall provide a sample contract. The sample contract must include all terms and conditions pertinent to this RFQ. Contract shall be in a form acceptable to the Board of Education. h. If respondent is currently under contract to the Monroe Township Board of Education, a copy of the current executed contract must be provided. 2. The Monroe Township School District intends to award professional services contracts for the defined scope of work under the Fair and Open Process in accordance with N.J.S.A. 19:44A-20.4 et seq. Submission of Proposals: Respondents shall provide evidence that the minimum qualifications have been met. Proposals should also include cost details and explanation where appropriate. Applicants will be notified if and when an interview is requested prior to an award.
