Vendor must include the bid title in the subject line The deadline to submit questions on this bid is 11:00 a.m. on the Thursday before the bid opening date. Answers to questions will be posted via an addendum no later than the Friday before bid opening Term of the agreement shall be for one (1) year with two (2) one-year options to extend the agreement at the sole discretion of the City of Omaha. Awarded bidder must provide detailed documentation for proposed price increases 30 days PRIOR to increase during the initial one (1) year term of agreement. Detailed documentation will be required for requested price increase to be considered for extension terms and will be based on the following: The total bid may be revised for an additional one-year term, and will not exceed the Consumer Price Index (CPI) established by the U.S. Department of Labor, Bureau of Labor Statistics. The CPI for the purposes of this contract shall be the most recent 6 month average, U S Department of Labor, Bureau of Labor Statistics, Consumer Price Index, All Items, Urban Consumers, applicable to the Midwest Region. Quantities listed are estimated annual usage only. Quantities may increase or decrease. All orders must be delivered within 72 hours of order being placed. Supplies will be ordered monthly. Individual products and quantity may differ month to month. There will be no minimum guarantee each month. These are minimum requirements only. The City of Omaha reserves the right to award the bid meeting minimum specifications or above. Estimated quantities are based on current case size stocked in current inventory.
