Renovation of an elderly care / assisted living facility in Medford, New York. Completed plans call for the renovation of a elderly care / assisted living facility.

The appropriate documents can be found in the "documents" tab of the advertisement. YOU MUST SUBMIT A BREAKDOWN WHEN YOU SUBMIT YOUR QUOTE. The breakdown must include the cost of materials, labor, and overhead. The project number is the CR NUMBER listed on the ad. Please include contract title in subject line of quote submission email. Selected vendor must already be assigned a New York State Vendor ID. If a vendor ID is not already assigned, selected vendor must obtain a NYS Vendor ID via submission of a substitute W-9 form to the contracting agency. Selected vendor must abide by the Standard Clauses of New York State Contracts. Selected vendor must provide up to date Certificate of Liability Insurance and up to date NYS Certificate of Workers' Compensation Insurance covering the period in which the service will be provided. A New York State Department of Labor Prevailing Wage Request is submitted on behalf of OPWDD as the contracting agency. This project will be assigned an Original Wage Schedule with an assigned Prevailing Rate Case number (PRC#). Selected vendor must comply with all prevailing wage regulations distributed by the NYSDOL Bureau of Public Work. Under New York State Labor Law, contractors and subcontractors must pay the prevailing rate of wage and supplements (fringe benefits) to all workers under a public work contract. Employers must pay the prevailing wage rate set for the locality where the work is performed. Prevailing wage is the pay rate set by law for work on public work projects. Any sub-contracted vendors must be approved by OPWDD Fiscal Services prior to services provided. Sub contracted agencies are subject to the same review as the primary contractor including required prevailing wage requirements. Selected vendor must not be on the NYS DOL Debarred List for the period in which service is to be provided. Any subcontracted vendors indicated must not be on the NYS DOL Debarred List for the period in which service is to be provided. The NYS Debarred List contains individuals and entities that have been debarred for violations of Articles 8 and 9 of the Labor Law, as well as for violations of the Workers' Compensation Law. OPWDD reserves the right to reject any quote and all quotes. Upon request, vendors must be prepared to demonstrate relevant experience on prior projects of similar magnitude. DESCRIPTION: (Replace counter tops and cabinets). The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 60 days of award, unless modified below. This is a NYS Public Works project and prevailing wages are to be paid. Refer to attached specification for the following scope of work. Site must be cleaned and all materials secured at the end of each work day. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of (Replacement), before removal of equipment and scaffolding, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent. Facility Address: 85A West Yaphank Road Coram, NY 11727 Office for People With Developmental Disabilities SCOPE OF WORK includes, but is not necessarily limited to the following: 1. House-A (side 1) Remove and discard cabinets and counters tops. New Cabinets and counters are to be installed in the same locations. One kitchen in each location must be operational during construction at all times. 2. Remove appliances and re-install to new work. 3. Provide and install oak Kraftmaid Shaker style cabinets and drawers, in color of Praline stain. Cabinets to be of all plywood construction, %" shelving, solid wood facing, soft close drawers and doors, toe kick and panels to match or (equivalent). 4. Kitchens will be fitted to accommodate a 30-inch wide stove and exhaust hood. 5. Supply and install new 10" deep stainless steel double bowl sink, new strainer, single lever delta faucet with sprayer, new supply valves and lines, new traps and piping, all connected to existing plumbing. 6. Supply and install new Corrian counter tops (color to be Sahara) with radius corners and rounded edges (for protection of Individuals) or (equivalent). Provide any supports for cantilever counters as needed. 7. Provide temporary counters tops until the new ones are installed. 8. Caulk and or seal all seam, corners, fixtures as required for proper installation to prevent water leaking and damage. 9. Contractor is to provide all patching and painting (color to match existing) in areas disturbed by project work. 10. Remove and dispose of all old materials and project debris off site. 11. Contractor to provide any third-party certifications or inspections. Contractor will supply a letter of compliance upon completion that work was completed as to scope and or plans, and to all codes and regulations.
