Pre-Qualified Contractor to extend an existing water main and provide a new Fire Hydrant pertaining to the City's Naugatuck River Greenway Phase I project. As part of the Naugatuck River Greenway Phase I project, the City of Waterbury is requesting proposals to extend the Connecticut Water Company's Watermain in South Main Street north to Platts Mill to provide water service to a Restroom being constructed near the corner of Platts Mill and South Main Street intersection, and to provide a new Fire Hydrant in the vicinity, all as described in more detail in the Special Provisions (also referred to in the Contract Documents as Technical Specifications) and Drawing for this RFP. The Selected Proposer (hereinafter "Contractor") shall extend the existing watermain with 8" D.I.P. provide valves, deadend blow offs, a 1" service line, a 6" hydrant service line and hydrant; conduct all necessary chloronation and testing as required by Connecticut Water, and repair pavement and replace loop detectors impacted by the proposed construction all as further detailed in the Special Provisions and Drawings included in the Proposal Documents for this RFP. The agreement period for any contract or purchase order resulting from this RFP is anticipated to be: The Contractor shall Substantially Complete all work and services required under this Contract within SIXTY (60) consecutive calendar days of the City's written Notice to Proceed and shall reach Final Completion within NINETY (90 consecutive calendar days of the City's written Notice to Proceed. The respondent shall provide insurance as set for in and required by Section 11 of the attached City of Waterbury Contract form (Attachment B). Insurers shall be satisfactory to the City and authorized to do business in the State of Connecticut, an "A-"Best's Rating and at least a Class V3 or better financial category as shown in the most current A.M. Best Company ratings. The Proposer to whom a contract is offered, must furnish to the City, if that contract has a total cost greater than FIFTY THOUSAND DOLLARS AND ZERO CENTS ($50,000.00), a 100 percent Performance Bond and a 100 percent Payment Bond each with a surety company acceptable to the City and in a form acceptable to the City. Proposer's attention is directed to Section 10 of the attached City of Waterbury Contract Form for additional information regarding required Performance and Payment Bonds. State of Connecticut Prevailing Wages will apply to this Project if a contract resulting from this RFP has a value of $100,000 or greater. Proposer's attention is directed to the attached wage rate schedules and documentation which may be applicable to this Project. Each Proposer shall submit a Proposal Security and include it as part of its Proposal submission. The Proposal Security shall be in the amount of ten (10) percent of the total of Proposer's "PRICE PROPOSAL for ITEMS 1.0 - 23.0," as set out in the attached Price Propsal Form for this RFP. The Proposal Security shall be either a Certified Check or Bid Bond. (See Attachment D for an example of Proposal Security acceptable to the City; however, a different form may be utilized provided it is acceptable to the City of Waterbury.) The City reserves the right to award in part, to reject any and all proposals in whole or in part for misrepresentation or if the proposer is in default of any prior City contract, or if the proposal limits or modifies any of the terms and conditions and/or specifications of the RFP. The City also reserves the right to waive technical defects, irregularities and omissions if, in its judgment, the best interest of the City will be served.
