All bids shall be plainly marked "Do Not Open - COTTAGE SHOWER ADA UPGRADES BID". All bids must have a fully completed Bid Form, 5% Bid Bond, Non-Collusion Affidavit, Business Relationship Affidavit, and proof of Indian ownership (if applicable). Refer to Section 00 20 00 for additional bid requirements. Pre-Bid Question & Substitution Closing Date May 25, 2022 - 05:00 PM - Submit in writing to diana@chahtaarchitecture.com A five percent (5%) Bid Bond, fully completed Bid Form, Non-Collusive Affidavit, Business RelationshipAffidavit, Payroll Affidavit, Non-Segregation Certification, Contract-Grant Certification and Tobacco & Drug Free Workplace Certification shall be submitted with the bid. If selected to enter into a contract, the Contractor will be required to provide a 100% Payment Bond, 100% Performance Bond, 100% Defect Bond as well as insurance coverage as described herein. Refer to Section 00 24 00 - Requirements for Insurance & bonds regarding additional requirements. Because the project is federally funded, the Contractor is required to comply with all provisions of the Davis - Bacon Act. Wage rate information for the project location is contained within this Project Manual. The Contractor is required to comply with related reporting requirements. Refer to the Supplemental Federal Conditions of the Contract. The Owner reserves the right to reject any or all bids and waive informalities and irregularities.
