. SCOPE OF WORK - STATEWIDE ELEVATOR MAINTENANCE SERVICES Vendor to provide monthly maintenance and emergency phone monitoring of elevators located throughout the State. Elevator maintenance and emergency phone monitoring services include, but are not limited to: o Manufacturers required monthly maintenance items, including all parts, labor, transportation, supplies, tools, software, hoists and safety equipment per State of Colorado regulations and American Society of Mechanical Engineers (ASME). o All replacement parts are to meet or exceed original equipment manufacturers (OEM) part specifications. o Vendor to submit a copy of their service plans with proposal. Plan should include but not limited to the following items to be covered: o Car positioning systems and controls o Power units, jacks, valves and pumps o Machine rooms o Pits o Car tops o Control circuits o Lubrication o Fluid changes o Adjustments o Safety Mechanisms o Vendor to inform owner of any deficiencies or recommendations found while performing work. o Vendor to coordinate State of Colorado required inspections. o Vendor to place required tags on all equipment and devices tested. Government jurisdictional tags will be affixed as needed. o All work to be warrantied for at least 30 days and parts shall carry a manufacturer's warranty. o Vendor to provide operations manuals and any software needed for the operation of elevators. o Vendor shall comply with all Local, State, and Federal Government Environmental regulations. o If the need for repair, or replacement parts beyond the scope of this agreement is determined during a Preventative Maintenance visit, the contractor shall notify the facility specific contact, or designee listed (this information will be provided to the successful bidder). Contractor shall provide a quote detailing labor costs including estimated hours, and itemized parts costs. DMVA approval, in the form of an approve change order to the Purchase Order, shall be obtained before work commences. o Any damage to the facility as a result of the service provider or their work will be the responsibility of the Contractor to repair as required, at the Contractor's expense.
