A Contractor desiring to respond to this Request for Quotations shall include a fully executed Non-Collusion Affidavit, and acknowledge all addenda (if applicable). The City of Sierra Vista has been awarded funding under The Emergency Home Repair and Disabilities Modification Program ("Emergency Program" or "Program") that assists homeowners in Sierra Vista to perform emergency repairs on substandard housing conditions. Substandard conditions include those posing a serious threat to the health, safety or welfare of the household or affecting the immediate livability of the home. The residence located at 125 E. Keating Street, Sierra Vista AZ 85635 has been determined eligible to receive such repairs under the program. The maximum amount of assistance per household, may not exceed $10,000.
