Bids will be received from contractors for work as shown on the Drawings and hereinafter specified and as set forth on the Contractor's Bid Form(s). Contractors need not bid every bid and every bid package, but Contractors must bid the alternates and unit prices within the bid package they are bidding. (Some bid packages have been divided due to their funding sources that cannot be combined.) 1.4 Bids will be opened publicly and read aloud at the Bid Opening, with a Bid Tabulation Sheet issued at a later date. 1.5 Contractors shall submit complete bid sets in triplicate, including one (1) original and two (2) copies of all submitted documents. Failure to do so may render the proposal incomplete or unresponsive. Contractors may initially preview the bid packets on-line at or during regular business hours at the Housing Authority's administration building, by making an appointment. Failure to do so may render the proposal incomplete or unresponsive. 1.8 The work for which Bids are requested includes all bonds, insurance, labor, materials, equipment, and items of materials and services required to provide the proper and complete construction of this project. The work shall be constructed in accordance with this Advertisement For Bids, Drawings and Project Manual as prepared for this project by Edward Revers of Revers Architecture and Design, NCARB certified, licensed to practice architecture in the State of Indiana. All other data required by Bid Form 96, Part II and Part III must be submitted as required by Bid Form No. 96. 1.11 Each individual bid package proposal shall be accompanied by a certified check or bank draft, payable to the Charlestown Housing Authority, Charlestown Indiana. U.S. Government Bonds, or a satisfactory Bid Bond executed by the bidder of an acceptable surety in an amount equal to five percent (5%) of the base bid plus all add alternates. 1.12 Sealed envelope shall list the name and address of the Proposal Bidder on the exterior of the envelope. 1.13 The successful bidder shall be required to provide a 100% Performance Bond and Labor and Material Payment Bond before starting any work or placing materials on the job. The Bonds shall be received within ten (10) days of receipt of Notice To Proceed. 1.14 All prices bid, are to be free of all Indiana State Sales Tax, and upon request of supplier, a tax exemption certificate will be executed by the Owner to the successful bidder to cover all items subject to this tax. 1.15 Attention is called to the provisions for equal employment opportunity, and payment of not less than the minimum salaries and wages as set forth in the specifications must be paid on this project. 1.16 Attention is called to the fact that the Contractor must take affirmative action to insure that applicants are employed, and that employees are treated during employment, without regard to their race, color, religion, sex or national origin. 1.17 Contractors are required to comply with the Provisions of Section 3 as set forth on 24 Part CFR B5 as follows: A. A good faith effort to utilize Section 3 area residents as trainees and employees. B. A good faith effort to award contracts for work in connection with the Project on Section 3 business concerns. 1.18 Attention is called to a request that the Contractor shall make a best efforts goal to achieve a state goal of 10% minority and/or women owned business enterprise participation in this project. 1.19 Bidders are required to submit the following documentation, in triplicate, with each individual Bid Package submittal: 1. Contractor's Bid for Public Works, State Form 96, as prescribed by the State Board of Accounts of Indiana, complete with Parts I - III, including the particular bid form(s) for which ever Bid Package (A, B, and/or C) that they are bidding. This completed form is a required and mandatory document for this project, regardless of the actual bid amount total. 2. HUD Form 5369A, Representatives Certification and Other Statement of Bidders. 3. HUD Form 2530, Previous Participation Certificate. 4. HUD form 5370, Contractor's Insurance Certificates with coverage and limits as stated in the General Conditions. 5. Stated acknowledgement of all addenda issued on the project. Contractor's Bid Guarantee: Bid Bond, Certified Check, or Bank Draft. 1.20 Bids may be held by The Housing Authority, City of Charlestown, for a period not to exceed forty five (45) calendar days from the date of the opening of Bids for the purpose of reviewing the Bids and investigating the qualifications of Bidders prior to awarding the Contract.
