Long Beach Unified School District ("District") would like to request a proposal from pre-qualified firms ("Firm(s)") to perform construction inspection ("Services") for the Asphalt Replacement Project at 4 district sites listed below: o Luther Burbank Elementary School (Burbank ES) - 501 Junipero Ave., Long Beach, CA 90814 o Whittier Elementary School (Whittier ES) - 1761 Walnut Ave., Long Beach, CA 90813 o Perry Lindsey Academy (Lindsey Academy) - 5075 Daisy Ave., Long Beach, CA 90805 o Cleveland Elementary School (Cleveland ES) - 4760 Hackett Ave., Long Beach, CA 90713 The primary goal of this project includes alteration to the hardcourt and parking lot areas which includes asphalt replacement, repavement, striping, and demo existing playground matting/equipment according to the bid plans and specifications. Construction Inspection services include, but is not limited to: o On-site throughout duration of the project. o Attend weekly meetings. o Attend project meetings, as necessary. o Prepare daily reports and semi-monthly reports o Act as liaison for District with regulatory agencies. o Document the construction progress with digital photographs and daily report. o Monitor progress and completion of as-built drawings. o Schedule and manage testing and inspection job activities. Coordinate any special inspections in accordance with the approved DSA T&I list. o Inspection duties and responsibilities as described CCR Title 24, Part 1, Section 4-342. o This is a DSA approved project. Consultant must comply with all DSA procedures and standards. o Implement requirements of DSA Box. o Verify all material delivered to the site relating to the project. o Review and assist the District in verifying contractor's payment applications. o Assist the District in the DSA certification and close-out of file process for the project.
