Projects to be bid per the following: Scope of work: Hot Mix Asphaltre- surfacing of Five Points Road and Whitcomb Road in Franklin County containing approximately 11,528 tons. The roadways will be bid and awarded as one project. Award will be within 15 days of bid opening. The Project must be completed within 90 days from the Notice to Proceed. Contrac- tors shall comply with all requirements of IC-5-16-13. Form 96: Total for all projects shall be listed on fonn 96. Actual award may be less than this total. Fonn 96 can be obtained at the office of the County Engineer. Completed Itemized Proposal Fonn to be attached to Fonn 96. Bid Bond; Performance Bond; Payment Bond: See Special Provisions Insurance: Workmans Compensation and mini- mum $1,000,000 liability each occur- rence, $2,000,000 aggregate coverage must be certified to Franklin County. Liquidated Damages: Liquidated Damages as listed in section 108.09 shall apply per calendar day if the project is not completed within the specified period of time.
