PROJECT INFORMATION Project Location: Navy Lake Site Allatoona, 166 Sandtown Road SE, Cartersville, GA 30121. Project Description: Remove existing Septic Field and Furnish and Install New Septic Tank and Field. Magnitude of this project is $24,000-$29,000. B. SCOPE OF WORK The Contractor shall provide all services, including permits, materials, labor, equipment and installation required to perform the work described in this SOW, i.e. remove the existing leech field, pump down the existing septic tank, and furnish and install a new 1500 gallon Septic Tank and a new 500 ft Leech Field. A Map showing the work location of the Septic Tank and Leech Field is included in this SOW as Attachment A and a Site Plan is Attachment B. The contractor shall provide installation services in accordance with the manufacturer's specifications and in accordance with publications listed in Section C. Work shall include but is not limited to the following: 1. Contractor is responsible for the following items prior to starting construction: a. Verify and check all dimensions and equipment to ensure proper repairs and replacements are made. b. Verify and locate all existing utilities located on the property in the location of the work (water lines, sewer lines, gas lines, electrical lines, etc.) c. Obtain all required permits from Bartow County Environmental Health Department and schedule their required inspections and ensure their approval during construction. d. Clear out, remove and haul off any brush and small trees located in the area of work for installation of the new septic tank and leech field. 2. Contractor shall pump out the existing 1500 gallon septic tank and shall check the septic tank for any damaged components. Report any damaged equipment to the POC for possible replacement. 3. Contractor shall pump down the existing 1500 gallon septic tank and shall remove the existing leech field. Contractor shall dispose of the removed materials off the Navy Lake property at an appropriate dump site. Contractor shall dispose of the removed materials off the Navy Lake property at an appropriate dump site. 4. Contractor shall furnish and install a new 1500 gallon Septic Tank in series with the existing septic tank. The new tank shall be equipped with a filter, two risers and an access lid. 5. Contractor shall furnish and install approximately 500 feet of High Capacity Chamber System Leech Field located behind the Comfort Station on the hill side. Specific location of the leech field will be determined by the POC. 6. Contractor shall protect the existing facilities such as fencing, drives, walkways, equipment, landscaping, etc. as necessary while conducting onsite activities. Repair or replacement of any damage to the existing facilities shall be the responsibility of the contractor. 7. Contractor shall provide operating and maintenance instructions on the new system to the facility staff, including submittal of manufacturer's specifications to the POC. 8. Contractor shall provide a written warranty on all materials and labor for a period of one year (longer if standard for materials/equipment) from the date of acceptance by the POC. If the answer to the question in 1.) is yes, then an interested party (those affected by the action) can request review and reconsideration from the Wage and Hour Administrator (See 29 CFR Part 1.8 and 29 CFR Part 7). Washington, DC 20210 SITE VISIT: Contractor and representatives of the contractor shall examine the site and determine for themselves the existing conditions and general character of the site. Claims for additional costs due to conditions that could have been verified by site investigation will not be authorized. Contractor shall contact the POC to schedule the Site Visit.
