Scope of work: Cost for Line Item #1 (include total cost for items A-J) These are known factors. Vendors must invoice for actual cost incurred as described herein. A) Construct an onsite sewage system that meets the location and construction specifications of the Sewage Handling and Disposal Regulations (12VAC5-610-10 et seq., the Regulations), and all other applicable state and local laws, regulations, and ordinances. B) Construct the above onsite sewage disposal system in compliance with the James City County Health Department (JCCHD) Construction Permit 147-21-0032 at 3018 Chickahominy Road Toano, Virginia 23168 in the location shown on the permit. The permit contains additional conditions, notes, and information needed to construct the onsite sewage system. C) Within 48 hours of signature of agreement to construct the onsite sewage disposal system, the contractor shall have the contents of the existing 1000 gallons septic tank serving the subject property pumped by a properly licensed sewer hauler. The contents of the exiting septic tank shall be pumped every 14 days up to one week prior to construction of the repair permit. The contents of the exiting septic tank shall be pumped every 48 hour for one week prior to installation of the repair onsite sewage system. D) As necessary, provide or subcontract with a licensed plumber and electrician to provide plumbing and electrical required to convey the wastewater from the house to the onsite sewage system as required by the Regulations, and the Virginia Uniform Statewide Building Code (VUSBC). This includes obtaining or assuring that the owner obtains all permits and inspections as may be required by the local building authority in compliance with the VUSBC. E) As necessary, provide or subcontract with a licensed plumber to provide plumbing required to relocate the existing water line 10 minimum from the onsite sewage system as required by the Regulations, and the Virginia Uniform Statewide Building Code (VUSBC). This includes obtaining or assuring that the owner obtains all permits and inspections as may be required by the local building authority in compliance with the VUSBC. F) Abandon any unused component of the former onsite sewage system as specified by the JCCHD Construction Permit 147-21-0032. The existing septic tank shall be pumped by a properly licensed sewage hauler, the tank crushed in place, lime placed over the crushed tank, and the tank hole filled with clean backfill material. G) Obtain a final installation inspection from the system designer, Larry Madison, Onsite Soil Evaluator and assure that the onsite sewage system complies with the Regulations. H) Provide all required documentation to the JCCHD following completion of construction and obtain an Operation Permit for the onsite sewage system.
