The Town of Smithfield (Town) is soliciting written quotes to provide, if requested, individual Asbestos Reports for residential structures located at: o 915 East Street. o 908 Rand Street. o 907 Second Avenue. o 714 East Street. The Town is also in the process of preparing individual demolition/clearance bids for houses located at 714 East Street and 907 Second Avenue. Asbestos reports must be completed for each of these houses. For each house, perform a site inspection for asbestos containing materials in accordance with all rules, policies, and procedures of the North Carolina Department of Health and Human Services (NCHHS). 1. Take samples of materials and send to a qualified laboratory for examination. 2. Prepare an Asbestos Inspection Report of findings in conformance with 15A NCAC 19C.0601-0607. The report is to include at a minimum the following items: 1. Perform a full site inspection for houses to be demolished/cleared and site inspection of appropriate areas of houses being rehabilitated for asbestos containing materials in accordance with all rules, policies, and procedures of the North Carolina Department of Health and Human Services (NCHHS) 2. Take samples of materials and send to a qualified laboratory for examination. Bulk sampling of suspect ACM shall be conducted in accordance with the sampling requirements promulgated by the United States Environmental Protection Agency's "Asbestos-Containing Materials in Schools Rule" (40 CFR 763, Subpart E), commonly referred to as the "Asbestos Hazard Emergency Response Act" or AHERA regulations. 3. Prepare an Asbestos Inspection Report of findings in conformance with 15A NCAC 19C.0601- 0607. The report is to include at a minimum the following items: o Address of property and date of inspection o Conclusions/Summary of Findings o Table and/or floor plan showing location of samples and findings; o Laboratory Analysis Report of samples taken from structures; This notice is available in Spanish or any other language upon request.
