The Project includes the following Work: 1 LS Mobilization/Demobilization; 1 LS Video Taping of Project Area; 1 LS Erosion and Sediment Controls; 1 LS Traffic Control; 1,950 LF 8" PVC DR-14 Water Line; 2,500 LF 6" PVC DR-18 Water Line; 5,000 LF 6" PVC DR-14 Water Line; 2 EA Concrete Anchors; 230 LF 1" Polyethylene Service Tubing (Open Cut); 20 LF 1" Polyethylene Service Tubing (Bore & Jack); 45 LF 3/4 " Polyethylene Service Tubing (Open Cut); 165 LF 3/4 " Polyethylene Service Tubing (Bore & Jack); 1 LS Remove and Replace 15" Reinforced Concrete Pipe w/ 15" Corrugated Plastic Pipe; 1 LS Remove and Reinstall Existing Culvert; 16 EA 3/4 " Service Meter Setting (High Pressure); 3 EA Water Sampling Station Equipment; 2 EA Tie New 6" Water Line into Existing 6" Water Line; 1 EA 8"x8"x6" M.J.T. Tee Fitting; 1 EA 8" M.J.T. Gate Valve, Complete w/Box and Lid; 6 EA 6" M.JT. Gate Valve, Complete w/Box and Lid; 6 EA 6" M.J.T. Tee Fitting; 7 EA Fire Hydrant Assembly, Complete; 5 EA 1" Air Release Valve; 2 EA 6" Automatic Post Flushing Hydrant Assembly, Complete; 195 LF Gravel Driveway Repair; 40 LF Concrete Driveway Repair; 60 LF Stream Bank Slope Protection; 9 EA Stream Bank Stabilization; 18 EA Belted Silt Retention Fences; 3 EA WVDOH Rock-Lined Ditch Repair; 9,000 LF WVDOH Inspection Fees; 9,600 Reclamation of Disturbed Area and related appurtenances. The Work will be substantially completed within 30 calendar days after the date when the Contract Times commence to run, and completed and ready for final payment within 60 days after the date when the Contract Times commence to run. Liquidated damages shall be $1,000 per day. A two envelope system will be used. Envelope No. 1 must have the following information presented on the front: Name and address of Bidder Bid on Contract - Contract #1 - Enterprise Road and Henderson Run Road Waterline Extension Received by the Town of Elizabeth Envelope No. 2 labeled "Bid Proposal" shall also be placed inside of Envelope #1. Envelope No. 1 will be opened first and the Bid Opening Requirement items checked for compliance as outlined on the Bid Opening Checklist on page BOR - 1 of these contract documents. If such documents are found to be in order, Envelope No. 2 "Bid Proposal", will then be opened and will be publicly read aloud. If the documents required to be contained in Envelope No. 1 are not in order, Envelope No. 2 "Bid Proposal" will not be opened and the Bid will be considered non-responsive and will be returned to the Bidder. A bidder may not withdraw his bid for a period of ninety (90) days after the date set for the opening of bids. Bids shall be accompanied by a certified check or bid bond payable to The Town of Elizabeth in an amount equal to five percent (5%) of the base bid. Contractor is required to provide the Drug Free Workplace Certification in accordance with WV Code 21-1D-5. All Bidders are required to have a current West Virginia Contractors License per Section 21-11-11 of the West Virginia Code. Bids received after the scheduled closing time for the reception of bids will be returned unopened to the bidders. For all further requirements regarding bid submittal, qualifications, procedures, and contract award, refer to the Instructions to Bidders that are included in the Bidding Documents. The Town of Elizabeth reserves the right to reject any and all bids.
