Renovation of a mixed-use development in Bath, New York. Completed plans call for the renovation of a one-story above grade medical facility.

This soliciation is restricted to SDVOSB contractors for BUILDING 76 4TH FLOOR NORTH FLOORING REPLACEMENT The place of performance is the Bath VA Medical Center in Bath, NY An award will be made to the lowest priced Offeror in accordance with FAR 9.103 , who is deemed responsive, responsible, and offering a fair and reasonable price. Other factors that will be considered are listed below. See the attached Statement of Work with Minimum Technical Requirements, Evaluation Criteria, Wage Determination. The NAICS code is 238330, the Small Business size standard is $16.5 Million The project magnititude is between $25,000 and $100,000. A pre-offer site visit has been scheduled for Thursday, Aug 11, 2022 at 1:00 pm EST. Contractors shall meet COR in the Lobby of B76 at the Bath VAMC. No additional site visit will be scheduled. The deadline for RFIs will be no later than Friday, August 12th, 2022 @ 4pm. All questions related to this procurement must be submitted via email to Devan.Bertch@va.gov. RFQs are due to the CO Devan.Bertch@va.gov by Wednesday, August 17th @ 3pm. Offerors must be registered and certified online in http://www.vip.vetbiz.org and must be registered at http://www.sam.gov under NAICS code 238330 to be eligible for award. The following factors will be used to determine technical acceptability: Factor 1 - Past Performance Factor 2 - Experience with application of spray fireproofing, fireproof sealant and caulking Factor 3 - Price See Evaluation Criteria Document attached. Late offers shall be considered unresponsive. RFQs must be accompanied by the completed past performance questionaire and Request for Information form. These forms are both attached to this RFQ. *A pre-solicitation notice for this project was sent out under 36C24222B0068 but due to a system error the solicitation number was changed to the above listed 36C24222Q1081. * GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for healthcare furniture for VA Finger Lakes Healthcare System, Bath Facility for Building 76 4th Floor North Corridor and HopTel Flooring Replacement that meet the VHA specifications and high-quality threshold while integrating excellent environmental stewardship. This requirement includes VA approved flooring material ordering, delivery, flooring preparation and professional flooring and base cove installation. Flooring project includes removing mastic from existing terrazzo cove molding, removing mastic/prepping flooring area per manufacturing instructions and installing LVT in corridors and flocked flooring in HopTel rooms, and then install base cove to finish. Note that existing carpeting removal shall be done by VA in-house shops prior to the contract install scheduled time. The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. 2.0 PERFORMANCE REQUIREMENTS Project delivery and installation in Building 76 4th floor North is contingent on the existing engineering project Building 76 4th Floor Ventilation Contract to be complete in 4th Floor North for this contract to begin install. Duty hours will be M-TH 7a-5:30p. The Government recognized US holidays are: New Year's Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. The contractor shall: a. Coordinate and provide project management of all products and services to manage, order, ship, deliver and install new flooring from the manufacturer, per manufacturers specifications until final acceptance by POC Interior Designer. b. Contractor is required to have on site staff supervisor with OSHA 10 certification. c. This project can be done during normal working hours as there is no one moved into the space at this time. d. Provide maintenance and warranty information on products specified and professional installation. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
