Interested parties must fill out and return a Request for Criminal History for each person who will be involved in providing direct services to the seniors prior to Tuesday, August 30th, 2022. Criminal History forms are available in person, by mail, fax, or email from the COA office and are a requirement to qualify for consideration at the September 1st Antrim County Board of Commissioners meeting. Those contractors selected by the Commission on Aging Board of Advisors will be paid $30.00 per snow removal when using a plow only or $40 per snow removal using a plow or snowblower and shoveling/blowing one pathway to door and clearing the mailbox (maximum hour per client) following each 4 minimum of new snow accumulation. If the senior does NOT WANT their walkway and/or mailbox cleared and chooses no additional snow removal on the application, the contractor utilizing a snowblower will be paid $30 per snow removal on driveway only. It is understood that one company/business or individual cannot serve all areas requiring service. Therefore, contracts will be let to more than one contractor to satisfy geographical location requirements. Successful contractor(s) are required to provide Proof of Insurance Certificate for commercial general liability to include contractual liability, products and completed operations, independent contractor's coverage and broad form general liability endorsement or equivalent. Proof of Insurance Certificate for motor vehicle liability insurance with limits of liability of not less than $500,000.00 per occurrence, combined single limit bodily injury and property damage to include all owned vehicles, all non-owned vehicles and all hired vehicles used in snow plowing and snow removal. Proof of Insurance Certificate for workers' compensation or sign waiver of workers' compensation insurance. The COA Board of Advisors reserves the right to accept or reject any and all Letters of Interest or accept those that are in the best interest of the COA. Final approval will be on the agenda of the September 01, 2022 Antrim County Board of Commissioners Meeting.
