Recycling of Mercury Lamps, Devices and PCB Ballasts All prices submitted are to be on the form below in accordance with all terms and conditions set forth in this Request for Quotation. Prices quoted should be in unit of measure shown. Any award resulting from this RFQ will be made to the responsive, responsible vendor which offers the lowest price on an aggregate basis. If award is noted to be made on an aggregate basis, any vendor response that fails to include pricing for all items may be rejected. Prices shall be quoted F.O.B. Destination - inside delivery, freight included and shall be inclusive of all costs. Current and/or anticipated applicable fuel costs should be considered and included in the price quoted. Work must be completed within 7 days after issuance of purchase order or notice to proceed. The purpose of this Quotation is to establish a term contract for collecting services which include transportation and collection of RECYCLING OF MERCURY LAMPS, DEVICES AND PCB BALLAST for Central Solid Waste Facility. Central Solid Waste facility reserves the right to quote separately, and items/ services if deemed to be in the best interest of CSWF. This quotation supersedes any verbal or written quotation now in effect with Central Solid Waste Facility. Background: The Solid Waste Program collects mercury containing lamps and devices from residents, small business, and other agencies through its Household Hazardous Waste Collection program. Lamps and devices are stored and accumulated at one (1) Solid Waste owned facilities for eventual recycling. Frequency of Service: Central Solid Waste Facility site will require service typically once per month. This may be altered to be more or less frequent based on the volumes and by mutual agreement of both parties. Contractor shall respond within 72 hours of request for service. Term of contract This contract shall commence on October 1, 2022 and terminate September 30, 2023, with the option for four (4) subsequent one (1) year renewals. Renewals are contingent upon mutual written agreement. Pick-up Locations: Hazardous Waste Collection Center - 13130 County Landfill Road, Tavares, FL 32778 352-343-3776 Ext: 1684 Packaging Materials: All boxes and other packaging materials will be provided by the Contractor at no cost. Yes or No Shipping Containers for 8' and 4' lamps provided at No Charge Yes or No Documentation: Each pick-up event will have a separate shipping paper indicating date, gross weight of each material, and destination. Copies are be submitted to Central Solid Waste Facility at time of pickup. For purposes of payment tare weights of pallets shall be subtracted from gross weight shipped. Price: The amounts indicated are estimates for Central Solid Waste Facility generated materials only and no guarantee is given or implied. The Central Solid Waste Facility reserves the right to increase or decrease quantities based on actual need. No estimates are made for any other entity using this contract. Price shall include all costs for labor, transportation, FOB Destination ,materials, recycling, and documentation. There will be no additional charges for mobilization, labor or materials. Central Solid Waste Facility reserves the right to reject all Quotations and to waive any informality. State or County License
