Generally, the work includes patching and restoring terrazzo floors and walls. Bids shall include: 1. Properly completed Proposal Form in duplicate. 2. Bid Deposit in the amount of 10% of Bid. 3. Not less than the prevailing hourly wage rate as determined by the Illinois Department of Labor pursuant to Illinois RevEventsised Statutes, Ch. 489, Sec. 39s-1 et. Seq. All Bidders are advised that all work ofthis Contract shall be subject to the provisions of the "Illinois Prevailing Wage Act". Prevailing wage rates have been determined and are on file at the Office ofthe State of Illinois Department of Labor, telephone (217) 782-6206. All Bidders shall submit and enclose with their bid anotarized certification that the Contractor isnot barred from bidding on the Contract as aresult of aviolation of either Section 33E-3 or 33 E-4 of Article 33E, Public Act 85-1295 (Senate Bill 2002) effective January 1, 1989. Certification islocated in the Proposal Form, Section 00300, of this Project Manual. Bids shall be accompanied by abid bond inthe amount of 10% of Bid, made payable to the Jacksonville School District. Failure of Bidders to submit bid bond, certified check or cashier's check in amount to cover proposals bid upon is cause for rejection ofbid. Successful Bidder will be required toprovide Surety Performance Bond and Labor and Material Payment Bond, each equal to 100% ofthe Contract cost on prescribed forms within ten (10) days after notice of acceptance is issued by Owner. Surety on bond shall be by adulyauthorized U. S. Treasury Backed company licensed to do business in the State of Illinois, acceptable toOwner. The Board of Education for Jacksonville School District #117 reserves the right to reject any or all bids and reserves the right to waive anyirregularity in bidding, which in their considered opinion is warranted. Nobid shall be withdrawn for sixty (60) days after opening of bids without written consent ofthe Owner.
