**No other documents available for this project** Work shall consist of removal, replacement & reclamation of approximately 400 concrete slabs, that are 5'x 5'x 4" in size on Strathmoor Blvd., Tennyson Ave., Winston Ave., Whittier Ave., and Byron Ave. Construction and disturbed areas shall be restored to pre-existing conditions by backfill, seed and straw. Public streets shall be kept clean of debris at all times. Staging area will be provided & all dumpsters & related equipment shall be located there when not in use. Best safety and security measures shall be practiced at all times. Bids shall include the price for one 5'x 5'x 4" concrete slab. Work shall be performed Monday through Saturday 8 AM to 5 PM, weather permitting. All work shall be completed within 90 days of contract award. Liquidated damages of $500 a day shall be applied for failure to complete work within the allotted 90 days. If the contractor is unable to meet the goals, they must demonstrate adequate good faith efforts. Bids shall include an estimate of time/days needed to complete the scope of work, a 5% Bid Bond, 100% Performance & Payment Bond, and a Certificate of Liability Insurance. A retainage of 10% will apply.Inclusion of MBE & WBE subcontractors is encouraged.
