The undersigned hereby proposes to furnish to the Board of Water Works Trustees of the Des Moines Water Works, the estimated requirement of vaporized liquid carbon dioxide of approximately 3,662,928 pounds for the year 2023. This vaporized liquid carbon dioxide will be used in the purification and softening of the municipal water supply. DMWW reserves the right to conduct any test/inspection it may deem advisable to assure goods and services conform to the specifications. Approximately 2,702,160 pounds will be delivered to: Fleur Drive Treatment Plant 408 Fleur Drive Des Moines, IA 50321-1190 Truck deliveries to Fleur Drive will be weighed before and after product is delivered. All deliveries will be made Monday through Friday between 7:30 a.m. and 4:00 p.m. Bill of ladings for all deliveries must be e-mailed to phoenix@dmww.com with seal numbers noted prior to delivery of product at Des Moines Water Works. All drivers must call our guard station 1 hour prior to delivery 515-283-8740. SDS and Certificate of Analysis must be delivered with the load. Approximately 960,768 pounds will be delivered to: McMullen Plant 12223 SW Maffitt Lake Road Cumming, IA 50061 Vendor's delivering to the McMullen Treatment Plant at Maffitt Reservoir need to provide weight scale tickets for truck and trailer. All deliveries will be made Monday through Friday between 8:00 a.m. and 2:00 p.m. Bill of ladings must be e-mailed to phoenix@dmww.com with seal numbers noted prior to delivery of product at Des Moines Water Works. All drivers must call our guard station 1 hour prior to delivery 515-283-8740. As needed ASR very little usage located between the two main plants approximately pounds will be delivered to: Army Post ASR 5470 Willow Creek Avenue Des Moines, IA 50321 The undersigned hereby warrants that this product meets or exceeds the requirements set forth in the latest revision of AWWA Standard B510, Standard for Carbon Dioxide. This material shall be certified as suitable for contact with or treatment of drinking water by an ANSI accredited certification organization in accordance with American National Standards Institute/National Sanitation Foundation (ANSI/NSF) Standard 60, Drinking Water Treatment Chemicals-Health Effects. An affidavit of compliance must be provided prior to acceptance of the first delivery. This material shall have a carbon dioxide content of at least 99.5 percent by weight. Suppliers shall provide their quote F.O.B. point of destination as listed above. The price quotation shall be a firm price per pound of the delivered product. Deliveries shall be made by truck delivery. Des Moines Water Works will only guarantee a maximum of five (5) days notice prior to a delivery date. The estimated quantities specified above are given for informational purposes only. The amounts will vary with the actual demand. These quantities shall not be deemed to guarantee either a minimum amount or restrict the maximum amount to be furnished. Des Moines Water Works will only enter into agreements with vendors willing to maintain price stability for all of calendar year 2023.
