The undersigned hereby proposes to furnish to the Board of Water Works Trustees of the Des Moines Water Works, the estimated requirement of sodium hydroxide of approximately 91,401 gallons for the year 2023. The undersigned herby states this product meets or exceeds the latest revision of AWWA Standard. DMWW reserves the right to conduct any test/inspection it may deem advisable to assure goods and services conform to the specifications. Approximately 91,401 gallons will be delivered to: Saylorville Water Treatment Plant 6500 N.W. 26 th Street Des Moines, IA 50313 Suppliers shall provide their quote F.O.B. point of destination as listed above. The price quotation shall be a firm price per pound of the delivered product. Deliveries will be specified to the supplier on an as needed basis. Deliveries shall be made by tanker trucks with deliveries made Monday through Friday between 7:30 a.m. and 12:00 p.m. Bill of ladings for all deliveries must be e-mailed to phoenix@dmww.com with seal numbers noted prior to delivery of product at Des Moines Water Works. All drivers must call our guard station 1 hour prior to delivery 515-283-8740. SDS and Certificate of Analysis must be delivered with the load.
