Site work and paving for a sidewalk / parking lot in Cookeville, Tennessee. Completed plans call for site work for a sidewalk / parking lot; and for paving for a sidewalk / parking lot.

This request for bids is for a contract to furnish all material, equipment, supplies and labor to install sidewalk and flagpole pads with three flagpoles. For scheduling contact: Facilities Manager: Don Myatt, 615-797-9701, don.myatt@tn.gov This event is to bid for a one-time purchase for TN Department of Environment & Conservation. General Conditions 1) On-Site work to be performed between contract award, and October 30, 2022, unless an exception is granted by the Facilities Management Regional Manager. 2) All work shall comply with IBC, standards, regulations, and other applicable codes. 3) Contractor is responsible for all permits and associated fees. 4) Contractor is responsible for verifying all dimension. 5) Contractor shall keep the jobsite clean and safe and leave in a neat and tidy manner. 6) All color selection to be determined by the Park Manager. 7) All material substitutions must be pre-approved. 8) All materials shall be installed per manufacturer's recommendation and best practice. 9) All dimensions listed are approximate. Contractor is responsible for actual dimensions prior to ordering any materials or bidding. Description of Work New Work to be Provided and Installed or Performed 1) Grade and compact path to provide a suitable base so that the final sidewalk matches the existing building concrete pad. Dispose of excess material at designated location on the park. 2) Provide and install new 8-foot by 120-foot and 6-foot by 62-foot concrete connecting sidewalks, 4-inches thick with welded wire mesh reinforcement. Concrete to be 3000 psi with tinting and medium brush finish. Sawcut expansion joints every 8 feet. 3) Provide and install two 25-foot bronze anodized aluminum, commercial grade flagpoles and one 30-foot bronze anodized aluminum, commercial grade flagpole, all flagpoles to include a matching aluminum flash collar, gold anodized ball ornament, cast aluminum cleat, and external halyard. Flagpoles to be rated at a minimum windspeed of 113 mph and installed per manufacturer's recommendations with a Schedule 40 sleeved or manufacturer's provided base with hidden lightning ground arrestors. Install flagpoles in a triangular layout with 10-foot on center. 4) Provide and install a level 20-foot diameter circular concrete pad centered on flagpoles. Concrete to be 4-inches thick, 3000 psi, welded wire mesh reinforced, and tinted with medium broom finish. 5) Provide seed and straw to all disturbed areas to re-establish grass vegetation. Additional Conditions 1) This is a turnkey job. All aspects of this job shall be left in a finished condition: All finish work, interior and exterior walls and trim, finish painting, caulking and final cleanup are included in this contract. 2) All bidders are advised to visit site to verify all conditions and dimensions. No allowances shall be made by the agency due to any bidder neglecting to visit the site and verifying dimensions and conditions. 3) Contractor shall be responsible for determining where all utilities are on the job site and care should be taken to protect the utilities from any damage caused by the demo/construction. This shall include any underground utilities around the job site area. If damage occurs, it shall be repaired within a 24-hour period from the time damage occurs. 4) Contractor shall perform work on regular time and shall invoice work time and material not to exceed the quoted price. Any variance in quote shall be addressed with a representative of Facilities Management, Middle TN Regional Office (MTRO) before additional work or materials are supplied. 5) Work shall be scheduled to avoid any interference with normal operation of the park as much as possible. During the construction period, coordinate construction schedules and operations with the agency. Work shall be conducted during the normal business hours of Monday through Friday, 8:00 a.m. to 4:30 p.m., unless approval for an alternate schedule is arranged with Facilities Management. 6) Successful contractor to schedule and attend a pre-construction conference where a preconstruction form shall be signed by Facilities Management, Contractor and Park Manager or park representative before work can begin.
