Replace and Automate Double Doors 1. Project Scope of Work Overview: Task 1: Contractor shall provide all tools, materials, components, labor, supervision to Automate the Double Doors leading to the Pain Clinic, Damen 5W. Contractor shall perform all work outside of normal hours of operation. Task 2: Contractor shall provide all tools, materials, components, labor, supervision to remove the existing doors and replace the pair of Fire Doors located next to room 5648, 5E Bed Tower. Contractor shall perform all work outside of normal hours of operation. Task 3: Contractor shall provide all tools, materials, components, labor, supervision to remove and dispose of the failed Operator and replace with new Double Doors leading to the Taylor Street Dock. Contractor shall perform all work outside of normal hours of operation. Task 4: Contractor shall provide all tools, materials, components, labor, supervision to Automate 3 sets of Double Doors inside the Dental Clinic, 4th floor Damen Bldg. Contractor shall perform all work outside of normal hours of operation. 2. Detailed Scope of Work: The Contractor shall provide and install the following: Task 1: New Dual Egress Operator (1) Electrified Panic Exit Devices (2) Isolation Relay Module (1) Motion Sensors (2) Door mounted swing safety sensor and controller (2) Touchless Actuators (2) Prove functionality to the POC before leaving station. Task 2: Remove and dispose of the existing doors Double egress fire rated steel doors with wood veneer to match as close as possible to existing doors (2) Electrified panic exit devices (2) Fire Rated Vision lites (2) Electrified Panic Exit Devices (2) Concealed electrified continuous (roton) hinges (2) Kick plates (4) Astragal (1) Rixson Floor Closure Cover Plates (2) Prove functionality to the POC before leaving station. Task 3: Remove and dispose of the existing failed Operator New Dual Inswing Operator (1) Door Mount Swing Safety Sensors and Controllers (2) Mag Lock with Bond Sensor/DPS Switch (2) Prove functionality to the POC before leaving station. Applicable Performance Standards 1. All work shall comply with all VA Policies including and at minimum: VA Construction Safety and VA Infection Control Policies. 2. The Prime Contractor shall always have a Competent Supervisory Person on site when any worker(s) or sub-contractors are present. 3. All Persons working on any electrical systems rated 50 volts or more shall have a minimum qualification as a State Licensed Master Electrician or Registered Journeyman Electrician, all activities shall follow the safe work practices in compliance with NFPA 70E Electrical Safety in the Workplace. 4. The Contractor shall provide mitigating activities. a. Fire extinguishers, pedestal mounted in the area of work b. No trash used packaging or construction spoils shall be stored on site. Remove each day using covered gondolas. c. Broom sweeps the work area at the end of each day. Provide HEPA vacuum and wet mop daily where broom sweeping creates undesired dust. 5. During work execution, the Contractor shall take care to protect Government Property Damage resulting from Contractor operations shall be repaired by the Contractors, including painting, refinishing, or replacement (if necessary), at no cost to the Government. 6. All work will be provided with a 1-year parts and labor guarantee from the date of acceptance by the Department of Veterans Affairs. 7. The Contractor shall be responsible for bringing all materials to site. There is no storage place available on site. 4. Jesse Brown VAMC Additional Requirements Contractor shall comply with all Jesse Brown VAMC site specific requirements and Standard Operating Procedures (SOP) including and not limited to: Key Policy, Badging Policy, Rules of Behavior, and Smoking Policy. 5. Hours of Operation 1. Normal work hours are 7:00 am to 4:30 pm Monday through Friday. Normal work hours can be modified only by written approval of the COR. The contractor shall not work on Federal Holidays or on the weekend when the Federal Holiday is on the respective Monday or Friday. a. Federal Holidays include: New Year's Day Labor Day Martin Luther King Jr.'s Birthday Columbus Day President's Day Veteran's Day Memorial Day Thanksgiving Day Juneteenth Day Christmas Day Independence Day Any other day specifically declared by the President of the United States to be a Federal Holiday. 2. If normal work hours are modified the Contractor shall check in (in person) with the Administrative Officer of the Day (AOD) which is in Patient Admitting for any work between 4:30 pm through midnight to 6:00 am or any time on weekends. Provide the name and cell number of the on-site supervisor to the AOD. At the completion of the work, check out with the AOD. 6. Check In/Check Out 1. Contractor shall present a valid form of identification at the Information desk to "Badge In". The information desk is located at the exterior entrance to Bldg. 2, Ogden Circle Drive Main Entrance. 2. Contractor shall notify VA Point of Contact (POC) upon arrival by phone 312-569-7574. 3. The VA POC will then escort the Contractor to the work area. 7. Documentation Invoices shall include the make and model number of the doors/operators/sensors installed and all work done. 8. Period of Performance Contractor shall complete all work including submittals, reviews, and quality control within 120 calendar days from the date of award. 9. Place of Performance Department of Veterans Affairs Jesse Brown VA Medical Center 820 S. Damen Ave Chicago IL 60612 Damen Bldg. 5W, Bed Tower 5E, Taylor Dock Corridor, Damen Bldg. 4th floor Dental The Contractor shall provide all tools, materials, components, labor and supervision to necessary to Replace and Automate Double Doors located at the Jesse Brown VA Medical Center. All work shall be performed in accordance with the Statement of Work (SOW) and Delivery Schedule.
