Provide City of Toledo with feasibility study documents and support for the Collins Park Stream Restoration Feasibility Study. Basic components include: site characterization, permitting, preparing multiple feasibility options, and public outreach activities. In order to be responsive, all proposals must have the following items with the submittal: project paperwork*; signed contract by the vendor*; child support affidavit*; Bureau of Worker's Compensation certificate; and Certificate of Insurance, naming the City of Toledo as additionally insured / Certificate Holder, and proof of all insurance as required by the Great Lakes Commission. All submissions must also include the proposal, working project schedule, and itemized costs.
