Scope 1) Remove and dispose of at contractor’s expense (to include providing your own dumpster): a) Approximately 150 feet of walls, 30 feet of drywall, 70 feet of demountable panels, and 30 feet of movable wall. b) Three office doors in rooms 1146, 1147, and 1148 c) One (1) Suite entry door adjacent to room 1148 d) One (1) pass through window in room 1111 e) All existing flooring throughout the space, with the exception of rooms 1105, 1108, 1137, and 1145 (including A, B, C, and D) f) Reception Desk and work table in room 1102. g) Cabinets and millwork in room 1145. The Washington County Facilities Management Office is seeking bids to remodel the Washington Ozaukee County Health Department office located at 333 East Washington Street Suite 1100, West Bend WI 53095. This project generally consists of the remodeling of approximately 4700 SF of office space in the Public Agency Building located at 333 East Washington St, West Bend, WI. It involves the removal and reconstruction of approximately 150 linear feet of walls, removal and installation of doors, flooring, cabinetry, and sink installation, as well as painting the office suite. Accepted bidder will be required to furnish an acceptable Certificate of Insurance upon award. All bidders shall be experienced in this type of work and will need complete and submit a Bidder's Statement of Qualifications in order to be pre-qualified with Washington County for this work. This completed form is to be received no later than 5 days prior to the bid opening. The Bidders Qualification Form is included in the bid document for this project. Additionally, bidders are to include with their bid three (3) references of similar projects completed within the past 3 years that Washington County may contact as well. Washington County reserves the right to waive any informalities or technicalities and to reject any and all bids or parts thereof deemed to be unsatisfactory or not in the Countyís best interest. Furthermore, Washington County reserves the right to cancel any order or contract for failure of the successful bidder to comply with the terms, conditions and specifications of the bid request and/or contract. Washington County reserves the right to award the bid in the aggregate or by item or like item groups (where applicable) to the lowest responsible, responsive bidder who complies with the specifications, service and can meet the requirements of this bid Final questions for this project are due by October 12 th .
