This RFQ includes the following documents: Scope of Work (SOW) Davis-Bacon Prevailing Wage Requirements Quote Information & Instructions: Any statement in this document that contains the word "will", "must" or "shall" means that compliance with the intent of the statement is required. Failure to satisfy that intent may cause the quote to be rejected. 1. All Quotes shall be in USD and in English language. 2. All charges must be included in the Quote including any associated costs for goods or services being delivered. 3. The Quote must include a timeline to complete SOW including a start and completion date. 4. Quotes shall be submitted via electronically to Procurement@hakc.org at the below designated date and time. Response must be clearly identified with the above project/work RFQ #22-1016 title as stated above. 5. Any Quote or questions MUST be sent via email to Procurement@hakc.org 6. Duration: The project work is to be completed in 15 calendar days, from the issuance of the Notice To Proceed. Liquidated damages of $125.00 per day, may apply for past due days. 7. HAKC is a sales tax-exempt entity. Copies of the sales tax exemption information will be provided to the successful bidder on request. The contractor shall not include in the bid amount, any taxes chargeable against the performance of the work. 8. Weekly Certified Payrolls are to be submitted be to procurement@hakc.org during the duration of the project Wages Project Work above $2000.00 USD require Davis-Bacon wages. Current wage decision can be found at online. HAKC has included a printed copy of the current decision in this RFQ. Project: Replace Roof Coverings, Gutter & Down Spouts Locations: 648 NE 83rd Street, Kansas City, Missouri 64118 Overview: 1. Remove & replace roof covering complete with all roof accessories to deck. 2. Remove & replace the entire plywood CDX roof deck. 3. Remove and replace gutters, down spouts, gutter guards, furnish and place concrete splash block under 2 down spout at south side. 4. Attach new down spout to existing buried 2 drainpipes at North side with matched down spout adapters, 5. Asphalt Roof Shingles shall be installed between 40 - 80 oF Temperature daily for 3 consecutive days. 6. HUD required Contractor to post Company Signage at the front of the house throughout duration of work. 7. Contractors are encouraged to attend Pre-Bid meeting to enable the bidders familiarize with all requirements for this work. 8. Notice to our Contractors: HAKC required Contractor to start work 1 week upon Contract award and issuance of NTP.. Preparation: 1. Remove roof coverings (possible two layers of asphalt coverings) & felts to deck. Include flashings around chimney, ridge cap, vents, other roof vents, gutters, gutter guards, down spouts, other roof covering/accessories and haul. 2. Remove the entire roof plywood deck and haul. 3. Call the Project Manager for Inspection and Approval after roof coverings are demolished to the bare deck. (Note: Contractor is required to inform the Project Manager as soon as shingles are removed to the deck for inspection and observe durability of the roof members). 4. Cut out & remove rotted fascia board as necessary around the roof. (20LF allowed) 5. Provide Unit Prices for removal and replacement of the following items with your Bid Submission: c. "LF" 1" x 8" Fascia, prime & paint d. "LF" 2" x 4" Rafters with Simpson metal structural tie connections e. "LF" 2" x 6" Rafter " " " " " e. "LF" 2" x 6" Joist " " " " Furnish & install the followings per manufactural manual installations instructions. 6. 5/8" or 1/2 " thick Plywood Roof CDX deck to replace the entire plywood CDX deck. (no particle boards allowed). (You are required to check existing deck plywood thickness on the pre-bid day) 7. Water Shield Felt Underlayment: ICE and Water Shield Roll Roofing Felt Underlayment (self-Adhered) at Eaves & Rakes use #30 Asphalt Underlayment on general roofs. 8. New Aluminum Square Roof Vents 4-6 on this roof. 9. New water protection 4" Vent Insulated Sewer and Plumbing stacks including flashings above the roofs. 12. 1"x 8" Wood Fascia, prime and paint. 13. Prime and paint fascia board around perimeter of the roof 14. (30yrs Warranty) Architectural Asphalt Roof Shingles to the entire roof area.: Use either Malarkey; Atlas; GAF Timbertex; GCP; Applied Tech; IKO brand that clearly shown (30years Guarantee) on the top & edge of the Shingle Bundles cover. (Color: WEATHERED WOOD). ** Submit Warranty Certificate for roof shingles replaced. (See General Requirement as outlined) 15. Roof Ridge Caps: Cap must be the same manufacturer with shingle, color, texture, and grade. Use Ventilated Ridge Cap where originally installed. 16. Aluminum/galvanized metal flashings include drip edges, flashing around chimney, flashing plumbing Stack Vents, pipe jacks and other roof moisture or water penetration barrier protectives around the roof to meet the City of Kansas City Dept. of Codes Administrations requirements. 17. Replace rusted Furnace Flue Vents (B-vents) and flashing above the roof complete. 21. Aluminum, Stock Unit Gutter 6" wide "K" type, 0.032" thick, White or match existing Gutter color. (No Gutter Segments, all gutters must be one piece at each elevation) (Slope gutters toward down spout directions with adequate gradients at both sides) 22. Aluminum 3"x 4" Down Spouts at the same locations where down spouts were removed around the roof Specification for Down Spout Installation: All down spouts required an extension with an elbow 24" long and slope away from the foundation wall. Properly placed down spouts over a concrete splash block. (Elbow of extension to be free 2-1/2" high from the ground) a. Turn down spout's extension end to the sides where water will flow away without back-up or any obstructions. 23. 6" Wide, E-Z Lock Small Holes Steel Powder Coated Screen Gutter Guard over the Gutters around the roofs. Follow Manufacturer Specification & Instructions for Installation and Properly Attached each Gutter Guard under the Roof & lock with gutter. 24. 48" long precast concrete splash block to each down spout and sump pump drains outlets. Remove any existing broken concrete splash blocks or plastics and haul 25. Station your dumpster to where it would not obstruction the resident movement 26. Remove construction debris irrespective of their sizes. Use magnetic roller and remove nails and any metal objects on daily. basis Roof Construction General Notes: 1. Remove all demolished shingles and other roof debris irrespective of their sizes. Use magnetic roller to remove nails and other metal objects on the ground. 2. Provide ladder for inspection during the progress of work and final inspection. 3. Provide Tarp covering to cover the entire roof in case unexpected rains or showers from demolition day thru covering installations. 4. Contractor is required to have a foreman or project representative on the site during the term of the contract
