Demolition, site work, renovation and addition to a municipal facility in Hazard, Kentucky. Completed plans call for the addition of a municipal facility; for the demolition of a municipal facility; for the renovation of a municipal facility; and for site work for a municipal facility.

There will be two contracts for the related work. The Phase 1 Contract will be with the City of Hazard, Kentucky. The Phase 2 Contract will be with the Perry County Ambulance Authority, Inc. There will be only one (1) Contract A Bid Form (the Total Project). The work for the total project will consist of site improvements, electrical improvements, mechanical and plumbing improvements and various architectural improvements and new architectural work. The project will be phased. Phase 1 of the project shall construct all detailed site improvements and the required improvements to allow the construction of the majority of the new first floor interior spaces. Phase 2 of the project shall include all work necessary to allow the construction of the new second story of the interior spaces and limited work within the existing first story area. All work for Phase 1 shall be completed first with Phase 2 work to follow the completion of this work. The OWNER reserves the right to waive any informalities or to reject any or all bids. The Owner also expressively reserves the right to delete certain portions of the project should available project funds be exceeded by the best bidder. Each BIDDER must deposit, with his bid, security in the amount, form and subject to the conditions provided in the Information for Bidders. Additionally, Bidder shall submit with their Bid a 'Certificate of Incumbency' if they are incorporated and shall submit those appropriate documents listed in the Information for Bidders. Attention of all bidders is particularly called to the requirements as to conditions of employment to be observed and minimum wage rates to be paid under the contract. Attention of Bidders is also particularly called to the requirements as to Section 3, Segregated Facility, Section 109 and E.O. 11246 and Title VI. Minority bidders are encouraged to bid. No bidder may withdraw his bid within 90 days after the actual date of the opening thereof.
