Project Category Code: Sewer Force Main, Water/Wastewater Infrastructure, Directional Drilling, Directional Bore The Project generally consists of, but is not limited to, the following: Contract # 138-2022-02 - Approximately 4,038 L.F. of 10-inch sewer PVC force main by open-cut methods; Approximately 370 L.F. of 12-inch sewer HDPE force main by horizontal directional drilling methods; Approximately 340 L.F. specially lined 10-inch DIP force main in casing pipes; Approximately 210 L.F. 18-inch casing pipe installed by boring methods. Bids will be received for a single prime Contract. Bids shall be on a lump sum and unit price basis, with additive alternate bid items as indicated in the Bid Form. Prospective Bidders shall provide a Submittal with their Bid containing the following information: (1) Evidence of two similar projects of at least $400,000 value completed in the previous 10 years, including detailed description, bid amount, final contract amount, scheduled time of completion, actual time of completion, Owners contact information, and Engineers contact information. Description of any lawsuits or claims the Bidder has been party to in the previous 10 years including resolution; (2) Number of employees directly employed by the Bidder; (3) Resumes or description of the proposed Project Manager and Project Superintendent including name, relevant experience, and employment history with the Bidder. Bid security shall be furnished in accordance with the Instructions to Bidders. The Contract Time is one hundred eighty (180) calendar days. The Contract(s) will be awarded to the lowest, responsive, responsible bidder. If proposal(s) is forwarded by mail, the envelope containing the proposal shall be placed in a separate envelope and marked as follows: Sealed Bid for Contract # 138-2022-02 Conference Center Force Main Relocation; Not to Be Opened until 2:00 P.M. The Bid shall be mailed to HWEA at 401 East Ninth Street, Hopkinsville, KY 42240 allowing sufficient time for such mailing to reach the address prior to closing time for receipt of Bids. HWEA may consider any Bid not prepared and submitted in accordance with the provisions of the Advertisement Contract Documents, and/or the Specifications to be non-responsive.
