For the replacement of the asphalt parking at the High Point Public Health Department, located at 211 S. Centennial St. High Point, NC 27260. Participation of minority-owned and women-owned businesses is encouraged. A Bid Deposit is NOT required for this Project. Guilford County reserves the right to reject any or all bids and to waive informalities. An Affidavit (C) that includes a description of the portion of work to be executed by minority businesses, expressed as a percentage of the total contract price, which is equal to or more than the 10% 1.1 DESCRIPTION A. The High Point Public Health Department is located at 211 S. Centennial Street, High Point NC 27260. B. The project consists of removing existing asphalt and installing new pavement over the existing base. C. Contractor/Installer shall comply with all federal, state, and local laws, rules, codes, and regulations, including OSHA. 1.2 SCOPE OF WORK A. Provide a quote price for all labor, taxes, permits, material, tools, equipment, and supervision necessary to fully complete the construction of replacing the asphalt parking lot for High Point Public Health (Approximately 75,000 Sq. Ft.) as follows: 1. The parking lot work can be performed during regular work day hours in sections(approximate 25% work area at a time) to allow remainder of parking lot to be in use. After hours and weekend work will be allowed with prior knowledge to the Guilford County Project Manager. (See Picture "Pavement Outline") 2. The roadway leading to the parking deck will need to be performed on weekends or after hours during the work week to allow access to the parking deck. 3. Cordon off work areas with barricades and channeling devices. 4. Remove existing asphalt from site and haul off daily, no accumulation of asphalt is allowed on site. 5. Proof roll the existing stone base with a loaded tandem dump truck to ascertainthe existing stone base and sub-grade is in a stable and firm unyielding condition per Section 2.0 6. Undercut and backfill areas that are insufficient. Unit Price per cubic yard is required. 7. Prep existing stone base for new asphalt pavement per Section 2.0 8. Installation of New Asphalt Surface Course per Section 2.0 9. Installation of New Pavement markings including all parking lines and ADA markings (See Picture "RHA Parking Lines", "Public Health Parking Lines") 10. Clean up and demobilization from site. 11. Any damages to existing curbing, sidewalks, or landscaping will be the responsibility of the contractor and repaired at their expense. 12. A (1) one-year warranty material and labor warranty must be included, and the warranty date will start at date of acceptance by Guilford County. 13. This will be turnkey project as all materials, labor, consumables, trash removal and disposal, spoil removal to accomplish the scope of work is the responsibility of the contractor. 14. All measurements are to be field verified by Contractor prior to bidding.
