ADDENDUM 1. This Request for Proposal (RFP) W25G1V-23-R-0006 is for the services associated with the Lightning Protection Services for Tobyhanna Army Depot, Tobyhanna PA 18466, in accordance with (IAW) the attached Performance Work Statement (PWS). 2. This solicitation is in accordance with FAR Part 12, Acquisition of Commercial Items and FAR part 13.5 Simplified Procedures for Certain Commercial Services. Simplified procedures for soliciting competition, evaluation, award documentation and notification that comply with FAR 13.1 will be utilized. This requirement is being solicited as a Total 100% Small Business Set Aside requirement. 3. The services to be performed fall under the authority of the service contract act unless otherwise exempt by the Contracting Officer. Wage Determination No: 2015-5797, Revision No. 21 dated 12/27/2022, Pennsylvania, Monroe County, is provided at Attachment III to this solicitation. 4. To receive an award, the company must have an active registration in the System for Award Management (SAM) https://www.SAM.gov with no active exclusions and have no debt subject to offset. Prospective offerors must be found to be responsible in accordance with FAR 9.1. 5. The minimum acceptance period for offers is sixty (60) days from the date specified in block 8, on page 1 of the (SF)1449, including any extensions of the offer due date by solicitation amendment. 6. BASIS OF AWARD Award evaluation and basis for award will be performed in accordance with FAR 52.212-2, Evaluation-Commercial Items, located in the clause section of the solicitation. The Government will award a hybrid firm-fixed-price and time and materials contract, resulting from this solicitation, to the responsible offeror whose offer, conforming to the solicitation, will be the best value to the Government. The Government reserves the right to award to other than the lowest priced offer. Offerors are cautioned that an award may not necessarily be made to the lowest price offeror. The award will be based on the best overall (best value) proposal that is determined to be the most beneficial to the Government. Three (3) evaluation factors: Price, Past performance, and Technical capabilities will be used. 7. PROPOSAL SUBMISSION REQUIREMENTS A. Contractor Price Proposal (See Solicitation Attachment II - Pricing Sheet for completion) B. Technical Proposal - this must show the Contractor possesses an Underwriters Laboratories Lightning Protection Master Certification of Certification from the Lightning Protection Institute (as per section 2.6 of the PWS), and in addition, must show the contractor meets all Technical requirements as listed in the Performance Work Statement. C. Past Performance to include 2 relevant contract samples within the past 5 years. D. Signed 1449 as noted above E. All Signed Amendments noted above F. FAR Clause 52.204-26 and 52.204-24 Must be filled out in the SF 1449 Solicitation (these are highlighted in yellow within the Solicitation). 8. PROPOSAL SUBMISSION INSTRUCTIONS AND REQUIREMENTS: A.
