UCFS Healthcare, Norwich, CT is seeking bids for the installation of vinyl long plank for the main of our Rock Nook building. UCFS Healthcare is a nonprofit 501(c)(3) that has served Eastern CT since 1877. Designated as a Federally Qualified Health Center in 2014, UCFS' current scope of project includes Primary Medical Care, including Pediatrics, Adult and Geriatrics; Women's Health/Gynecology; Dentistry and Dental Hygiene; and, Behavioral Health including Counseling, Psychiatry, Substance Abuse treatment and Addiction Services including the state's largest Gambling Addiction program. This RFP is the property of UCFS and must be held in strict confidence. The RFP contains information regarding UCFS operations and is provided solely for the purpose of submitting a proposal. The Bidder shall not disclose to anyone, other than those who need to know in order to respond to this RFP, any information concerning this RFP. Bid should include the following: Remove and dispose of existing carpeting and padding. Prep existing floor as needed (will include the removal of wax on existing floor) Supply and install Patcraft Commercial Luxury Vinyl Plank Floor (or equivalent) (color to be determined) Supply and install 4" vinyl cove base on all walls where new flooring was installed (color to be determined) Supply and install matching vinyl transition moldings as needed in doorways/entrances Clean, haul and dispose of all debris (not in UCFS dumpster) Proof of Insurance Vendor shall purchase and maintain in full force defect insurance policies with the limits of insurance provided below. Commercial General Liability with limits not less than $1,000,000 per occurrence, $2,000,000 general aggregate, $1,000,000 and $2,000,000 products-completed operations aggregate. Such insurance shall cover liability arising from premises, operations, independent contractors, product-completed operations, personal and advertising injury and liability assumed under an insured contract. Business Automobile Liability with a limit of not less than $1,000,000 each accident, including owned, non-owned, leased and hired vehicles. Statutory Workers Compensation & employers liability coverage for all employees, including corporate officers and sole proprietors. Umbrella/Excess Liability with a limit of not less than $1,000,000 per occurrence/aggregate.
