For: Henry Street Park Borough Of Fairview Bergen County, New Jersey Project Location: 102-118 Bergen Blvd, Fairview, NJ 07022 Bids will be rendered in accordance with Chapter 107 N.J.S.A. Bids must be plainly marked and the envelope containing the bid shall be endorsed on its face with the name of the person, firm or corporation making such proposal and the bid number and name for which such proposal is made. The Borough will not be responsible for the premature opening of any bid not so marked. FAXED proposals do not qualify as written sealed bids. Each bid shall be accompanied by the information listed in the Proposal Forms including the following: Bid Guarantee - Guarantee payable to the Borough of Fairview that bidder, if the contract is awarded to him, will enter into a contract therefore. Guarantee shall be in the amount of 10% of the bid, but not in excess of $20,000 and may be in the form of certified check, cashiers check, or bid bond. Consent of Surety - Certificate from a surety company stating that it will provide the bidder with a performance bond in such sum and form as is required in the specifications. Successful bidder will be required to furnish a Performance Bond in the amount of 100% of the contract and satisfactory in form, execution and sufficiency of surety. All bidders must comply with the requirements of N.J.S.A. 10:5-31 et seq. and N.J.A.C 17:27 An Act relating to Affirmative Action. All bidders must submit a photo copy of their Federal Letter of Affirmative Action Plan Approval or a photo copy of their Certificate of Information Report or a completed Affirmative Action Employee Information Report (AA302 or AA201) after notification of award. NJ Contractor's Registration Certificate must accompany bid packet. Contractors and their subcontractors shall comply with the requirements of the New Jersey Prevailing Wage Act, Chapter 150 of the Laws of 1963, N.J.S.A. 34-1156.25, etc., regarding payment of prevailing wages to workmen engaged on the project. All bids must be on the form of proposal, must include all other affidavits and information required, and must conform to the specifications. No bidder who is on the State Treasurers, The New Jersey Economic Development Authoritys or the Federal Government's List of Debarred, Suspended or Disqualified Bidders shall be eligible to bid on this project. All bids will be opened and read to the public at that time. No bid may be withdrawn for a period of sixty (60) days from the opening of the bids. The contract, if awarded, shall be awarded to the lowest responsible bidder. No bid shall be deemed accepted until adoption of a formal resolution. Project consists of construction of Henry Street Park. The extent of the construction is shown on construction drawings and these specifications include, but are not limited to: Demolition: Removal of existing asphalt, base course, vegetation, and fencing. - Removal of existing asphalt and base course. - Site grading. - Removal guide rail and post. - Removal drop curb and sidewalk Parking Lot: (Parking lot and driveway) - Level site at new Pedestrian Park. - Provide new bollard and pole lighting and furnishing at the Pedestrian Park. - Remove and replace the existing asphalt and base course to necessary grades. - Provide concrete driveway. Site Work: (Outdoor Fitness, and Construction of Pedestrian Park) - Remove of existing playground. - Site grading. - New trees planting. - Landscaping and mulch. - Provide retaining walls, curbs, and fencing. - New walkways, and ramps. - Install path lighting and furnishing. - Construction of new Outdoor Fitness Area. - Construction of new parking lot with 1 inlet and associated piping to tie into and existing yard inlet. - Installation of arborvitaes adjacent the neighboring property lines - Installation of modular block retaining wall. - Security lights installation on CCTV camera and poles. - Installation of a new inlet and underground stormwater pipe and connection - Storm lines associated with connecting the new stormwater management system to the existing drain line as shown on the plans. - Installation of cabinets and concrete pad to house electrical equipment outdoors adjacent to Pedestrian Park. Modular Block Retaining Wall: - Site grading. - Provide retaining walls, curbs, fencing, and railing. - New walkways, steps, and ramps. - Installation of paths lighting, and furnishing. - Landscaping and mulch. - Water service line from proposed backflow preventer to the park and new water service as shown from Henry Street. - Storm lines associated with connecting the new stormwater conveyance system to the existing drain line as shown on the plans. - Contractor shall provide signed and sealed plans for the proposed Modular Block Retaining Wall. The cost of the engineered plans shall be in included in the modular block wall bid item. - Contractors shall include fence with privacy slats in Bid Item for the modular block retaining wall. All related incidental work, including traffic control, dust control, soil erosion control, clearing, cleaning and restoration. The above Scope of Work outlines the general items and shall not be construed as being all inclusive.
