Kingsley Ave Water Main Improvement The Project site is located within existing street right of ways for both City and FDOT roads. The project limits run from Holly Leaf Lane to Kingsley Avenue on Doctors Lake Drive and in Kingsley Avenue from Doctors Lake Drive to Plainfield Avenue. The Project limits are within the Town of Orange Park. The major work elements include, but are not limited to, the following: Bid: a. Removal of existing asphalt and concrete as needed to install new water main b. Connect to existing 6" water main at Holly Leaf Lane. c. Connect to existing 12" water main at Doctors Lake Drive at Kingsley Avenue. d. Connect to existing 12" water main in 2 locations at Kingsley Avenue at Plainfield Avenue e. Install new 6" PVC (DR-18) water main pipeline f. Install new 8" PVC (DR-18) water main pipeline g. Install new 12" PVC (DR-18) water main pipeline h. Bacteriological clearance and pressure test new water mains i. Reconnect existing customers to new water main j. Replace asphalt and concrete removed during construction. The cost of the plans and specifications from CMT is $150.00 (non-refundable) and will be provided electronically in PDF format. The project will also be available directly from the Town of Orange Park on their Demand Star site for digital download as well. Bidders must submit with the Bid, documentation that they have constructed similar type work completed during the past five years with references. Each bidder must include resume and qualifications for their proposed on-site supervision superintendent. Said Superintendent shall have maintained continuous employment with bidder for a minimum of three years. The bidders will be required to submit a Bid Bond equal to 5% of the bid with their Proposal. The successful bidder will be required, upon award, to furnish a 100% Performance and Payment Bond, with a Surety acceptable to Owner. Each bid proposal shall remain binding for at least 90 days after the scheduled bid date. The Owner shall reserve the right to accept or reject any or all bids/proposals, waive minor formalities, and any that are in any way incomplete or irregular. The Owner further reserves the right to reject all bids/proposals and re-advertise, or to reject all bids/proposals without further action and award the bid/proposal that serves the Owner's best interest.
