23-MX-BB-003 is an SBR project for a contractor to furnish and install a 2-part epoxy flooring system in the bathroom at the Department of Juvenile Services, Backbone Mountain Youth Center, 124 Camp 4 Road, Swanton, MD 21562. Small Business Reserve Designation Yes VSBE Participation Goal 0% SCOPE OF WORK A. This project consists of the Contractor providing all labor, equipment, tools, materials, supplies, insurance, licenses and permits, etc., necessary to install approximately 805 square feet of a 2-part epoxy flooring system (Pharma floor) as well as approximately 198 linear feet of wall that is 11' high, (approximately 2178 sq. ft). Walls include 8" cove base. There are 5 toilet stall walls 4X4 and 2 knee walls 5X4 (approximately 200 sq. ft.) to be covered also. B. Contractor shall prepare floor by removing any existing Epoxy that is loose by grinding/sanding existing concrete floor/walls and 8" cove base on block and concrete walls/floors per manufacturer specification. C. The contractor is responsible to remove the existing bathroom fixtures during floor installation. The fixtures are - 5 toilets, 1 urinal, a 3 bay sink, a single sink and 3 showers. When the floor is completed, the contractor shall reinstall all same fixtures. D. The contractor will remove the metal step transition strips from 6 steps and epoxy area, then attach metal strips back to steps. E. As part of the contract, if the Contractor finds excessive damage, beyond the normal damage of the concrete. Grinding/removal of damaged concrete and repairs will be determined by the DJS Maintenance Supervisor or designee, the Contractor shall provide a Unit Price to repair 100 sq. ft. of concrete repairs, to include materials and labor as stated in specifications. F. Contractor shall provide appropriate material to join new flooring with existing flooring at each doorway. G. The epoxy flooring system shall have the color and texture to be determined by DJS Maintenance Supervisor or designee. H. The epoxy flooring shall be minimum 1/4" thick. It shall be applied to the prepared area(s) strictly in accordance with the Manufacturer's recommendations. The finished floor and wall coating system shall be uniform in color, texture, and appearance. All edges that terminate at floor discontinuities and other embedded items shall be sharp, uniform, and cosmetically acceptable with no thick or ragged edges. I. Provide system data sheets in compliance with requirements of United States Department of Agriculture (USDA), Food and Drug Administration (FDA) and local Health Department. J. It is the Contractor's responsibility to verify all measurements and calculate quantities prior to bidding. K. Contractor shall make sure all existing plumbing floor clean outs are level with new floor. L. The Contractor shall provide an emergency phone number for warranty repairs.
