Statement of Work Project # 623-20-126 Re-roof Bldg. 50, 53, 56 Roofing Project Jack C. Montgomery VA Medical Center - Muskogee, OK Scope of work: The project has a requirement for the replacement of existing roof system which includes re-roof with new TPO roof system. Investigation will take place to replace support surfaces and structural elements as needed, replace all perimeter and penetration flashings, remove all unnecessary penetrations replace all roof drain hardware, rings, gutters, and downspouts In addition to the re-roof, Bldg. 53 will require removal and replacement of the lightning protection system. Refer to page EJ132 of the design and section 26-41-00 of the VA specifications for lightning protection plans. Due to the Very Severe Hail (VSH) classification of Muskogee and the surrounding area, a 115-mil fleece backed TPO membrane (60 mil TPO + 55 mil fleece) and high-density coverboard was selected. The Bldg. 50 roof is sloped metal deck, approximately 460 square feet. The Bldg. 56 roof is sloped structural concrete deck, approximately 2,100 square feet. Building 53 is the largest roof and includes an elevated penthouse roof. The main roof is a flat structural concrete deck, approximately 32,650 square feet, with tapered insulation for drainage. The Machine Room roof is a sloped metal deck, approximately 2,650 square feet. All roofs will have the aggregate, roofing membrane, and insulation removed down to the structural deck. New polyisocyanurate insulation, high-density coverboard, and 115-mil fleece backed TPO membrane will be installed in all locations. The concrete roof decks will receive a vapor barrier below the insulation and the metal decks will have a substrate board to cover the fluted deck. General: Contractor to provide all required labor, material, equipment, and site investigations to develop to perform re-roof of Bldgs. 50, 53, and 56 as called out in design drawings and specifications. Firms shall be properly licensed and bonded to perform general contracting work in the State of Oklahoma or have equivalent licensing and be approved for such work by the Veteran s Administration. It is to be documented in the construction documents that the prime construction contractor shall provide engineering and technical support, all manuals, wiring diagrams, and tools necessary for adjusting, maintaining, repairing, and testing of equipment for use by VA Engineering staff or VA s designated Maintenance Provider Construction documents to include contract drawings and specifications for the development of contract documents. Project Management: Based on the above list of work items, contractor must complete, maintain, and submit the following documentation and managerial requirements: Construction schedule including sequencing and durations. The schedule will be provided to the Contracting Officer s Representative (COR) fifteen calendar days after the pre-construction meeting Site-Specific Safety Plan (SSSP) with an emphasis on tasks to be performed during the work. The SSSP must: Include a detailed description of how the work will be performed and be submitted with sketches, supporting documents, submittals etc. Include a Job-Safety-Analysis (JSA) for all tasks associated with Statement of Work (SOW) Be reviewed and commented by VA-Safety and COR prior to commencement of work. Maintain site supervision by a competent individual(s) with OSHA-30 certificates. OSHA-30 certified technician shall be onsite anytime work is being performed. Obtain VA identification PIV badges for all employees. Contractor to consider lead-time for badge procurement. Manage job-site access and security. Implement and maintain required ICRA and ILSM measures. Code and Regulation Compliance: The Contractor shall comply with public laws and statutes including all changes and amendments to federal, state, and local environmental statutes and regulations in effect the date of issuance of this delivery order, including applicable portions of the documents cited in the basic contract and below: NFPA 99-2015 Healthcare Facilities Codes. SSSP to be reviewed and commented by VA Safety staff and COR prior to start of construction activities. Construction Demolition/Removal Plan Inspection reports, manufacturer approval certificates, and warranties as noted in specifications. Penetration and or hot work permits as necessary. Manufacturer and construction warranties as noted in specifications. Project Schedule including sequencing and durations. SOV Schedule of Values Site superintendent OSHA-30, and tradesmen/laborer OSHA-10 certificates. OSHA-30 certified technician shall be onsite anytime work is being performed. Material Submittals Submit for COR approval Manufacturer s specifications for all materials and equipment planned for installation. Inspection reports, manufacturer approval certificates, and warranties as noted in specifications. Technical literature and/or descriptions of all materials, equipment and tools proposed for use by the Contractor shall be submitted to the Contracting Officer or his/her designee for approval prior to contract start and whenever a change is required. All Contractor-furnished materials, equipment and tools used shall be standard products all manufacturers regularly engaged in the production of such items. The Contractor shall secure all materials. The Government assumes no liability for loss of or damage to Contractor-owned property held upon Government premises. Contractor shall submit hours worked, full report of completed piping which shall include location, hours worked, the full name of Contractor s employees on site, and digital photographs before and after work is completed. Reports shall be submitted within one day upon completion of work performed. Contractor will provide a completed redline drawing of the completed routing of piping. If necessary, a secondary report shall be submitted after any repairs outside of the Work Required have been completed. Reports shall be provided in hard copy as well as in an agreeable and compatible electronic format such as, but not limited to, pdf or Microsoft Word. The Contractor shall assume responsibility of environmental protection of all material storage. The VA safety officer will hold VA medical center specific safety training. All contracted personnel will be required to have this course Any personnel found working on-site without it will be asked to vacate the premises immediately All contractors deemed a supervisor, superintendent, or any variation of shall have an OSHA 30 card and provide proof at the VA medical center safety training. Proof of safety training and OSHA 30 cards shall be electronically submitted to the COR and CO. All contracted personnel shall have a minimum of OSHA 10 certification and shall provide proof during the VA medical center training. Hours of Work: With few preapproved exceptions, the hours of work shall take place during the normal business hours of 0800-1600 (8:00 AM to 4:00 PM), Monday through Friday. Federal Holidays excluded. Weekend, after-hour, and Federal holiday work is permitted with a request in writing submitted beforehand. Allow 10 working days for approval. Off hours are not to billed as overtime. Federal Holidays and any other day declared a federal holiday by the President of the United States are defined below. The Contractor and COR will inspect the work. Deficiencies will be noted and corrected before beginning work in another area. Any damage done by Contractor will also be noted and corrected by the Contractor within 2 working days. Damage causing an immediate impact on the environment of the hospital shall be corrected immediately. Quality control measures will be observed daily by VA safety staff. Acceptance Standards: The VA reserves the right to dispute completion of contract scope if contract work is found not in accordance with manufacturer recommendations, or is not satisfactorily executed in accordance with the SOW. All work completed by the Contractor must be reviewed and approved by the COR prior to final acceptance. Meetings: The contractor is required to attend the pre-construction meeting after award, this meeting will be held at a date and time specified by the Contracting Officer to review project scope, site, and requirements, as well as provide answers to any questions. Badges and Parking Contractor must acquire PIV badge for all staff onsite prior to work beginning. While on VA Facilities official I.D. badges shall be worn continuously. Failure of the background check may result in specific personnel being disallowed to work on VA premises and the Contractor will need to provide replacement personnel. Contractor shall coordinate all badging requests through the COR It is the responsibility of the Contractor s personnel to park in the appropriate designated parking areas. The designated parking area will be specified by COR upon award of contract. While performing under this contract, all vehicles used by the Contractor shall be locked and the keys removed when not in use. This is intended to protect the Contractor s property and the safety of the patients at the Medical Center. Parking passes and/or PIV badges can be accepted and/or rejected by the VA police force. The VA police force is a federal law enforcement agency. Access The COR shall provide the Contractor access to all buildings and rooms required in the provisions of this contract. The Contractor shall notify the COR when leaving these areas and immediately turn in any keys to prevent unauthorized reproduction or utilization. Contractor shall give the COR 48 hours' notice when access to a locked space is needed and granted. *The value for this project is based on a financial range.
