Per addendum 1 bid date extended to 3/6/23 no change in time. 2023 Terre Haute CCMG Project All bids must be submitted on the prescribed State Board of Accounts Bid Form No 96, which includes the completed execution of a non-collusion affidavit and proposal form included in specifications. Bidders must be INDOT Certified to submit. Bid Security: Any person, firm or corporation who submits proposals MUST file with their proposals a Bond or Certified Check in the amount of 5% of said bid made payable to the Board of Public Works and Safety. To ensure faithful completion of the contract entered into with the City of Terre Haute by the successful bidder, said bidder will be required to give a Perfor- mance Bond and Payment Bond due at the time of the execution of the contract, in the full amount of the contract price with sufficient surety thereon. The Board of Public Works and Safety reserves the right to reject any or all bids presented and waives technicalities as to procedures and to award a contract on the bid that, in its judgment, is the most advantageous to the City of Terre Haute, Indiana. All bidders must submit a fi nancial statement. A unit price contract will be awarded for the construction of the 2023 Terre Haute CCMG Project illustrated on the associated plan sheets. The project consists of milling and repaving Johnson Drive from S 3rd Place to S US 41; milling and repaving Johnson Avenue from 3rd Street to 4th Street, milling and repaving Hospital Road (4th Street) from Davis Avenue to E Hospital Lane; milling and repaving 7th Street from 400 feet S of E Davis Drive to E Royse Drive; milling and repaving Davis Avenue from 45 feet E of S US 3rd Street to 315 feet W of S 7th Street; milling and repaving Fruitridge Avenue from 445 feet S of Wallace Avenue to 990 feet S of E Margaret Avenue; milling and repaving Walnut Street from S 3rd Street to S 19th Street; milling and repaving Wabash Avenue from N 35th Street to 420 feet W of Woodridge Street; milling and repaving Locust Street from N 25th Street to N Brown Avenue and milling and repaving Locust Street from N Brown Avenue to 30 feet W of Heritage Lane E. The plans depict the scope of work for the project. All work must comply with the City of Terre Haute Standards and INDOT Specifications where applicable, or as specified in technical specifications, and the requirements of the Americans with Disability Act where applicable. Bidders shall submit State Board of Accounts Form 96 (Questionnaire and Financial Statement). Bidders shall submit a list of similar projects completed.
