The County of Volusia has forty-five (45) elevators, four (4) escalators, and two (2) chair lifts that require annual inspections. For the list of locations, see Exhibit C. The County of Volusia is soliciting sealed responses for Contractor to furnish annual elevator inspections. The County has forty-five (45) elevators, four (4) escalators, and two (2) chair lifts that require annual inspections. A. Annual Inspection Contractor shall perform annual safety inspection of all elevators, escalators, and chair lifts once per year to include the following: 1. Coordinate the repair of all discrepancies noted during annual inspection with the County of Volusia elevator maintenance contractor. The current elevator maintenance Contractor is Oracle Elevator Holdco, Inc. The elevator maintenance contractor(s) is subject to change during the term of the agreement resulting from this Request for Quote (RFQ). 2. Re-inspect all noted discrepancies after repair. 3. Coordinate and witness all required periodic tests mandated by the State of Florida or other governmental agencies. 4. Prior to expiration of current inspection certification, provide certified inspection reports to the State of Florida for each piece of equipment inspected with copies of the same to the County Project Manager. 5. Any elevators with two stops only require Maintenance Verification, however, the County reserves the right to request inspections. Contractor shall be responsible to notify the elevator maintenance contractor who will be responsible for submitting said verification to the State of Florida. B. Qualifications / Requirements 1. Contractor shall be a state certified QEI-1 Elevator Equipment Inspector. If this certification is allowed to expire or is revoked during the period of this agreement, the Contractor shall notify the County Project Manager immediately and the agreement may be terminated. 2. Contractor shall provide copies of state certifications to the County of Volusia. 3. Contractor shall not be affiliated with County of Volusia elevator maintenance contractor(s) in any way. Contractor shall be a third-party independent inspection service. C. Documentation Failure of the Contractor to provide to the State of Florida any documentation which results in the equipment covered under this agreement being removed from service may result is a fine from the State of Florida. The Contractor shall be responsible for the reimbursement to the County of the complete cost of any fine from the State of Florida.
