The work consists furnishing all labor, materials and equipment for repairs to the exterior of an existing 300,000 gallon elevated water storage tank at Water Treatment Plant No. 2, as described in the Contract Documents. All work shall be performed in accordance with the Contract Documents. The project site is at the following location in Duval County, Florida: Water Treatment Plant No. 2, 2301 Mayport Rd, Atlantic Beach, FL 32233. Proof of Experience: Bidders shall submit proof of minimum 3 years of experience with selected similar projects. The company must be registered and permitted to do business in the State of Florida. It shall have been in business and have a record of successful continuous operation for at least three years under the same name and tax identification number. Qualifications: Bidders shall submit proof that they are currently licensed as a General Contractor. Proof of Insurance: Bidders shall submit Certificate of Insurance, naming the City of Atlantic Beach as Certificate Holder, showing the bidder has obtained and will continue to carry Workers Compensation, public and private liability and property damage insurance during the life of the contract. Failure to complete and return these required forms with the bid submittal will make the bid nonresponsive and provide a basis for rejection. All Bids must be made on the forms provided, properly executed, placed in an envelope and mailed or delivered in accordance with this Invitation to Bid. Bid Bond in the amount of 5% of the bid A Performance and Payment Bond will be required of the successful bidder. Bid prices must remain valid for ninety (90) days after the public opening of the bids. Goods and services proposed shall meet all requirements of the Ordinances of the City of Atlantic Beach. ANTI-COLLUSION REQUIREMENT: Under no circumstances shall any prospective proposer, or any person or persons acting for or on behalf of any said prospective bidder, seek to influence or gain the support of any member of the City Commission or the City Staff favorable to the interest of any prospective bidder or seek to influence or gain the support of any member of the City Commission or City Staff against the interest of any prospective bidder. Any such activities shall result in the exclusion of the prospective proposer from consideration by the City. A person or affiliate who has been placed on the convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to provide any goods or services to a public entity, may not submit a bid on a contract with a public entity for the construction or repair of a public building or public work, may not submit bids on leases of real property to a public entity, may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity, and may not transact business with any public entity in excess of the threshold amount provided in Section 287.017, for a period of 36 months from the date of being placed on the convicted vendor list.
