CHANGES/REVISIONS: 1. BID FORM A. Remove and Destroy: Page 18 of 19 "Bid Form" from the Bid Documents. Insert: The enclosed revised Page 18 of 19 "Bid Form" marked "Revised Addendum No. 1 into "Bid Form" of the Bid Documents. Note: Each Bidder shall use the Revised Bid Form page for submission of the Bids for this Project. Note: Changes are in bold italicized type. Description of Bid Item 5.B. has been revised to per ton. The project includes road milling and super pave material installation.
