Each bid must contain the full name of every person or corporation interested, and in case the bid is submitted by a corporation, a copy of the corporate resolution authorizing submission of the bid shall be furnished with the bid. No bid will be considered unless made on bid forms prepared by the Engineer, Donald F. Sheehy, Chagrin Valley Engineering, Ltd., 22999 Forbes Road, Cleveland, OH 44146. Said bids shall separately state the prices bid for labor and for materials and shall be submitted in accordance with the provisions of law, State and Federal, with reference to the employment of labor on public works. Each bidder is required to furnish with its proposal, a Bid Guaranty and Contract Bond in accordance with Section 153.54 of the Ohio Revised Code. Bid security furnished in Bond form, shall be issued by a Surety Company or Corporation licensed in the State of Ohio to provide said surety. Each Proposal must contain the full name of the party or parties submitting the proposal and all persons interested therein. Each bidder must submit evidence of its experiences on projects of similar size and complexity. Bidders must comply with the prevailing wage rates on Public Improvements in Cuyahoga County and the City of Olmsted Falls, Ohio as determined by the Ohio Department of Commerce, Bureau of Labor and Worker Safety, (614) 644-2450.
