Send the LEIN Request form, filled out and signed, by email to Daniel T. Smith at email address: SmithD76@michigan.gov. The email "Subject" must include (Facility Name, Project Name, Date & Time of Pre-Bid Walk Through Meeting). NOTE: An individual is only permitted to represent one bidder at a mandatory walk-through. DESCRIPTION OF WORK: Chimney height reduction, cap replacement, chimney liner installation, water heater vent duct replacement, chimney repointing/caulking/brick replacement as needed, chimney painting, roof/wall/site protection as needed, loading/hauling/legally disposing of unwanted materials related to project work. NIGP CODES: 91240 (Demolition Services); 91200 (Construction Services, General); 91455 (Masonry); 91054 (Painting); 91036 (HVAC). ACCEPTING QUESTIONS UNTIL: Wednesday, April 5, 2023 (by no later than 10:00 am ET)
