The contractor shall sequence his work to install new water service for residents. Other items of work include saw-cutting and restoration of existing pavement; replacement of curbing, sidewalk, driveway aprons and existing conditions in general disturbed in connection with this project as shown on the set of Drawings entitled "Rena and Franklin Street Water Service Replacement" . The location of this work is Rena Street extending from Pearl Street to Church Street and also on Franklin Street from Madison Avenue to the end of the roadway in the Borough of Newfield, New Jersey as identified in the Plans and Specifications.. This work is funded with a Municipal Bond Ordinance; appropriate regulations will govern. All bids must be on the proposal form attached to the Specifications in the manner designated therein, must be enclosed in sealed envelopes bearing the name and address of the bidder and the name of the project clearly marked "BOROUGH OF NEWFIELD WATER SERVICE REPLACEMENT RENA AND FRANKLIN STREET" and addressed to the Purchasing Board of the Borough of Newfield, Newfield, New Jersey. The bid must be accompanied by the following: Bidder's Checklist; Bid Proposal Form; Bid Guarantee in the form of a Bid Bond or Certified Check, or Cashier's Check in the amount of 10% of the total amount of the bid proposal, not to exceed $20,000, made payable to the borough of Newfield; Consent of Surety; Acknowledgment of Receipt of Addenda or Revisions; Stockholder Disclosure Certification; Business Registration Certificate issued by the N.J. Division of Revenue; Non-Collusion Affidavit; Public Works Contractor Registration; New Jersey Debarred List Affidavit; Indemnity and Hold Harmless Agreement; Affirmative Action Compliance Notice; and Bidder's Checklist.. This bid is being solicited through a fair and open process in accordance with N.J.S.A. 19:44A-20.5. Bidders are required to comply with the requirements of N.J.S.A. 10:5-31 et seq. (P.L. 1975, c 127, as amended and supplemented) and N.J.A.C. 17:27, Affirmative Action requirements of the State of New Jersey.
