Furnish tree trimming and tree removal services at the Aloha Stadium, 99-500 Salt Lake Boulevard, Aiea, HI 96701. Contract start and end dates provided in this solicitation are anticipated start and end dates. The Successful Offeror shall commence work upon the Stadium Authority's issuance of a Notice to Proceed. The following documents, forms and/or certificates must be submitted as an attachment on HIePRO: Offer Form OF-1, Offer Form OF-2, Wage Certificate, Arborist Certificate, and valid Specialty Contractor's License. The Offeror's Unit Bid Price and/or Total Bid Price must include all costs of labor, materials, delivery, surcharges, all applicable taxes including the current Hawaii General Excise Tax and any other expenses, including travel, and mileage.
