The City of Winston-Salem is seeking a vendor to provide case management services for City of Winston-Salem employees and subsidiaries, as deemed by contract. The successful bidder will be expected to provide face-to-face field case management services, for all appointments unless discussed with the Workers' Compensation Adjuster. This contract will not exceed $150,000 per year including documentation, travel, hearing testimony and any additional fees. The successful bidder to the RFP can expect an initial contract for a term of 2 years, with the option for the City to extend this Agreement for as many as two (2) additional twelve (12) month periods, provided that both parties are in agreement and funds are made available for this purpose.
