It is the intent for Mesa Unified School District #4 to seek the services of qualified contractor to supply Domestic and Irrigation Back Flow Systems Services: including (Inspection, Testing, Reporting, Maintenance and Repair) throughout the District. The first year of this contract is scheduled to cover the period from July 01, 2023 through June 30, 2024. Subsequent years for this contract would be based on the level of service, cost for future year(s), and the general impact to the District. The District estimates that it's spend is approximately $150,000.00 each year on Domestic and Irrigation Back Flow Systems Services. This is an estimated amount only and no actual amount is intended to be guaranteed or implied. Question Submission Deadline:June 1, 2023, 3:00pm Question Response Deadline:June 3, 2023, 3:00pm Contractor Selection Date:June 20, 2023 It is the intent of the Mesa Unified School District #4 to seek the services of a contractor to provide testing, maintenance and repair services for District wide internal, domestic and irrigation back flow devices. This RFP does NOT cover the testing and maintenance of fire protection back flow devices, which have been bid separately. The District is comprised of 200 square miles with approximately one hundred (100) schools and/or facilities. Presently, there are approximately 235 back flow devices that require testing throughout the District. A list of the most current back flow devices (provided by City of Mesa) is attached. Remodeling and/or construction of new schools may require adding additional units to the list. Awarded vendor must be willing to provide testing and repair service on any unit or accessories/parts the District allocates. Plumbing Lead, Randy Morrow, will oversee this project (phone # 480-472-6070 and email: remorrow@mpsaz.org) All work specified in this proposal shall be performed during the normal work hours of 7:00 a.m. to 4:30 p.m. (August through May) and 6:00 a.m. to 3:40 p.m. (June and July) except in cases of emergency situations. Emergency hours are defined as 4:31 p.m. to 6:59 a.m. weekdays, Saturdays, Sundays and holidays. As the District increases in number of schools and facilities, Mesa Schools may require the awarded vendor to service back flow devices at locations not listed in this proposal. RFP prices shall remain firm for any added back flow devices. Awarded vendor may be required to purchase repair/replacement supplies from companies other than those listed in this proposal. At the request of the plumbing lead, awarding vendor will be required to provide, in writing, any applicable percentage discount for manufacturers not listed herein. Vendor is to respond within five (5) working days to this request, when applicable. All internal, domestic and irrigation back flow devices shall be annually inspected and/or repaired (when applicable) and tested in compliance with city and state codes. Vendors receiving award shall be responsible for providing repair/replacement parts at the listed discount off of manufacturer's list price. Labor for repairs/replacement shall be limited to those listed within this RFP. A blanket Purchase Order will be issued each year to cover the costs of back flow testing and repairs throughout the District. However, be advised that if any repair exceeds the following repair limits (parts and labor), you must first receive written approval from the Plumbing Lead, Randy Morrow. 3/4" back flow devices $100.00 1" back flow devices $130.00 1 1/4" back flow devices $160.00 1 1/2" back flow devices $190.00 2" back flow devices $210.00 2 1/2" back flow devices $700.00 3" back flow devices $775.00 4" back flow devices $1,000.00 6" back flow devices $1,500.00 8" back flow devices $2,000.00 Should the purchase price of any repair/replacement part(s) exceed the dollars specified above, the vendor shall contact Randy Morrow, Plumbing Lead, 480-472-6070, who shall have the sole authority to approve additional expenditures by the awarded vendor. The Mesa School District reserves the right to determine if the cost of replacement parts, beyond the amount shown above, is reasonable and customary and shall, at the District's discretion, seek quotations from other vendors if deemed necessary to do so. The annual inspections for back flow devices referenced in this proposal hall be done in accordance with the schedules attached to this RFP. There may be times, however, when the District requires an emergency response to an unexpected problem with this equipment. In cases of emergency, the vendor agrees to respond to the emergency within four (4) hours from the time of the call. If the emergency is a result of negligence on the part of the vendor or their technicians, the service call will be at no charge to the District. In cases where equipment fails or is vandalized, and which are not related to vendor negligence, the District will be charged an hourly fee as specified in this proposal. Contractor must be able to provide annual inspection, testing and maintenance of all backflow devices defined in this proposal. Contractor must hold a State of Arizona Commercial Contractor License, CR-05 or C-37/CR-37 to inspect, test, report, maintain and repair back flow prevention systems included in this proposal. (Provide any other necessary licensing per authority having jurisdiction) Contractor must have a minimum of five (5) years' experience in the testing, maintenance and repair of specified internal, domestic and irrigation back flow devices. Contractor must employ a minimum of two (2) certified technicians at all times that will specifically be assigned to Mesa Schools to service the backflow devices contained in this proposal. Technicians must hold current certifications with ASETT or ABPA or AWWA ratings. Technicians must also have a minimum of two (2) years' experience in the field of back flow testing and maintenance and must have been fully certified during that two-year period. Vendor must include, in the Questionnaire section of this proposal, a list of technicians that will be servicing back flow assemblies. Attach a copy of each technician's certification to the RFP response. Certifications must fall within all Federal, State and City Requirements or any authority having jurisdiction. Contractor must have an emergency response technician in place. Contractor must service District back flow devices during the normal work hours of 6:00 a.m. to 3:30 p.m., Monday through Friday, unless otherwise directed by Plumbing Lead, Randy Morrow, 480-472-6070. Service shall not be done on Saturdays, Sundays and holidays unless previous arrangements have been made with Randy Morrow. Contractor must instruct technicians to wear proper clothing and company identification (a business card does not qualify as proper identification). Vendors are required to check in at each school or facility office before servicing and/or repairing back flow assemblies each time they are on Mesa Schools property. Contractor must provide repair/replacement parts that are the best of their respective kind. Contractor must comply with the schedule included with this proposal, or as modified, which coincides with the City of Mesa inspection schedule. Contractor must conduct business in a manner that insures complete safety to Mesa Schools students, employees and community members. Contractor must list total hours worked and parts used on the work request for each device that is tested/serviced. Vendor is to include location, model and size of device. Contractor must clean up work environment, leaving it in a neat, unobstructed condition, with equipment in perfect repair and surroundings in order. Contractor must complete testing and insure that all backflow assemblies are in full compliance with State and City codes prior to the due date specified on each annual inspection notice. Contractor must provide the City of Mesa Water Quality Services office with completed test results no later than the due date specified in each inspection notice. Contractor must provide Randy Morrow with copies of all completed test results on all devices within five (5) working days from the day each backflow assembly is tested and/or serviced. Test results are to be emailed to Randy Morrow at remorrow@mpsaz.org and Keith Bonkoski at kabonkoski@mpsaz.org or delivered to the Operations Facility, 555 South Lewis St., Mesa, Arizona, 85210. A quote for repair on systems must be emailed to the Project Manager for approval, within 48 hours. Contractor must return parts removed from back flow devices to the Mesa School District, if directed by the plumbing lead. All parts removed from back flow devices shall become the property of Mesa Schools and the District reserves the right to request that the contractor return any and all parts if requested. Invoice the Mesa School District in a similar manner to the following example: When replacement/repair parts are necessary, each kit, item, etc. shall be entered on a separate line of the invoice. List the school name, date of service, type of service, name of manufacturer, part number, manufacturers listed price, your company's discount as reflected in the proposal response and extended price. List any testing or labor charges separately. Note: Awarding vendor will not be expected to break out kits. The term "Repair/Replacement" applies to any major adjustments, upgrades and/or part replacements that may be needed to repair a system. Estimated annual expenditures are approximately $50,000.00 but no volume is implied or guaranteed.
